超声引导高危入路颈内静脉穿刺置管

目的：评价超声引导上腔静脉综合征患者高危入路颈内静脉穿刺置管的可行性及成功率。方法：30例上腔静脉综合征拟行静脉转流手术患者，随机分为两组，分别采用超声引导和传统盲穿法行高危入路颈内静脉穿刺置管，观察一次穿刺置管成功率、二次以上穿刺置管成功率、穿刺时间及并发症情况。结果：超声组与盲穿组的一次穿刺置管成功率分别为86．67％和53．33％，二次以上成功率分别为13．33％和26．67％，总体成功率分别为为100％和80．00％。并发症发生率分别为0．00％和20．00％。结论：对于上腔静脉综合征需行高危入路颈内静脉穿刺置管的患者，超声引导法成功率明显优于传统盲穿法，且穿刺时间明显缩短。

Ultrasound-guided catheterization of the internal jugular vein in high-risk routine

Objective： To evaluate ultrasound-guided catheterization of the internal jugular vein in high risk route in superior vena cava syndrome patients. Methods： 30 superior vena cava syndrome patients were divided into two groups. The internal jugular vein catheterization was performed by the ultrasoundguided and conventional methods respectively, observing the successful rate with one attempts and more than two attempts, puncture time and complications in two groups. Results： The satisfactory successful rate was 100% and 80.00% respectively in ultrasound-guided group and conventional group, 86.67% and 53.33% with one attempts, 13.33% and 26.67% with more than two attempts. The complication rate was 0.00% and 20.00% Conclusion： The ultrasound-guided technique offers both safety and convenience in catheterization of the internal jugular vein in high risk route.
