拉米夫定治疗慢性乙型肝炎停药后的临床观察

探讨拉米夫定药后慢性乙型肝炎患者临床变化的意义。对34例停用拉米夫定的慢性乙型肝炎患者,在停药1月、3月、6月时检测血清ALT、HBeAg、抗－HBe、HBVDNA,观察其转变情况。拉米夫定治疗前、后出现HBeAg至抗－HBe转变者为另一组（A组）,未出现ABeAg至抗－HBe转变者为一组（B组）,对比两组患者停药1月、3月、6月时ALT、HBV标志的变化。结果显示停药1月、3月、6月时ALT异常经累加后A、B两组分别0％和18．52％（P＞0．05）、0％和33．33％（P＞0．05）、14．29％和59．26％（P＜0．05）;HBVDNA阳转率分别为0％和25．93％（P＞0．05）、0％和44．44％（P＜0．05）、28．57％和77．78％（P＜0．05）。表明随着停药时间的延长。病情复发者增加。血清HBeAg至抗－HBe转变可作为拉米夫定停药的一个标志。

Clinical observation of chronic hepatitis B after lamivudine''s treatment

To explore the change of chroric hepatitis B patients' indexes after lamivudine's treatment. 34patients after the treatment of lamivudine were examined at 1, 3, 6 month(s), the indexes were ALT, HBeAg, Anti-HBe, HBVDNA, etc. These patients were divided into two groups(group A and group B)according to whether HBeAg change to anti-HBe or not. ALT abnormal rate of the two groups after 1, 3, 6 months were 0% and 18. 25 % (P > 0 .05),0% and 33. 33 % (P > 0.05), 14.29% and 59. 26% (P < 0.05); HBVDNA positive rate of the two groups were 0% and 25 .93% (P > 0.05). 0% and 44. 44% (P < 0. 05 ) .28. 57 % and 77. 78 % (P < 0. 05 ), respectively. After the treatment of lamivudine, the re- currence increased with the time, the change of serum HBeAg to Anti - HBe can be used as a symbol of ceasing drug.
