放射免疫显像诊断卵巢良恶性病变的研究

摘    要:
目的 :探讨卵巢良恶性病变放射免疫显像规律及在卵巢肿瘤诊断、治疗方面的价值。方法 :用131I-CEAMcAb放射免疫显像 (RII)诊断 4 4例临床拟诊为卵巢癌的患者 ,使用γ -闪烁显像辅以微机处理技术和双核素相减术。结果 :经手术和病理证实的卵巢癌 32例中 31例阳性显像 ,1例阴性显像 (假阴性 ) ,9例良性病灶均获阴性显像并经手术证实。 2 8/31例阳性显像的卵巢癌RII分期与手术分期完全相符。 91处转移灶发现 86处 ,阳性率 94 .5 % ,最小检出灶直径≥ 1cm。结论 :用此法在对病灶行定性诊断的同时可直接显示病灶的位置、形态、大小与数量 ,对早期发现卵巢癌 ,指导临床分期和制定治疗方案以及估计预后方面有较高的临床价值

Abstract:
Objective:To study the radioimmunoimaging(RII) for diagnosis of ovarian benign and malignant lesions with 131 I-labelled CEA monoclonal antibody.Methods:44 patients of clinical suspected ovarian cancers were studied by RII.32 patients were proved ovarian cancers by operation and histopathology.Results:31 of 32 cases were well localized by RII (true positives) and one false negative.RII staging of 28/31 patients with ovarian cancer were well agreed with surgical and histopathological stage.9 cases of benign ovarian tumors were proved to be true negative by operation.86 of 91 metastases were found in RII.The positive rate is 94.5% and the smallest tumor lesions≥1.0cm in diameter could also be detected.RII is more useful than ultrosonography and CT scanning in distinguishing benign tumors from malignancies.The location,size and number of the lesion can also be found directly. Conclusions:RII of ovarian cancer with 131 I-CEA McAb is valuable and helpful in detecting early ovarian cancer and directing clinical staging and treatment.
