厄贝沙坦联合卡维地洛对高血压患者昼夜节律及心脏功能的影响

目的观察厄贝沙坦和卡维地洛联合治疗高血压的效果。方法300例高血压患者,根据动态血压结果,按照夜间血压下降率<10%为非杓型组,≥10%为杓型组。给与卡维地洛和厄贝沙坦联合治疗6个月。观察治疗前后动态血压和心脏结构及功能的变化。结果高血压存在着明显的昼夜节律。非杓型组与杓型组相比左心室质量、左心室质量指数及左心房内径、舒张功能障碍明显增加。治疗后,非杓型组昼夜节律恢复,心室质量、心脏舒张功能均有明显改善。结论卡维地洛和厄贝沙坦联合可以恢复高血压患者的昼夜节律,减轻心肌重塑,改善舒张功能。

The Effect of Irbesartan Combination with Carvedilol on Circadian Rhythm, Cardiac Structure and Function in Patients with Hypertension

Objective To investigate the effect of irbesartan combination with carvedilol in treating patients with hypertension.Methods 300 patients with hypertension were divided into two groups according to their lowering rate in the night by ambulatory blood pressure monitoring(<10% as nondipper group,≥10% as dipper group).All patients were given irbesartan combination with carvedilol for 6 months.Cardiac structure and function were examed before and after treatment.Results Cardiac rhythm was found in all patients.LVM,LVMI and LAD were significantly higher in nondipper group than in dipper group.The diastolic function of left ventricle was significantly damaged in nondipper group than in dipper group.After 6 months'treatment,the cardiac rhythm was recovered,LVM,LVMI,LAD and diastolic function of left ventricle were improved.Conclusions Irbesartan combination with carvedilol can resume the cardiac rhythm,relieve left ventricle remodeling and improve left ventricle diastolic function.
