药物运输泵与难治性癫痫

摘    要:癫痫患者耐药机制各不相同,其中运输泵机制能限制抗癫痫药物到达癫痫发作部位而引起耐药。运输泵中最典型的是P-糖蛋白(P-glycoprotein,P-gp),主要表达在一些有分泌功能的器官和血脑屏障中,能限制亲脂性药物进入脑内。抗癫痫药物在理论上似乎可以是P-gp的底物,过表达的P-gp降低了这些药物在癫痫灶内的浓度从而引起耐药。这一观点为难治性癫痫的治疗带来了新的希望,如运用一些运输泵的抑制剂来提高脑内抗癫痫药物浓度。

Abstract:Epileptics have their own mechanism of drug fast in which one is caused by drug traneportor who can restrict the anti-epileptic drug to its site of action.The most typical transporter is P-glyeopretein expressed in many organs having excrctive function and blood-brain barrier which can restrict lipotrupy drug to brain. Antiepileptic drugs seem to he the substrate of p-glycoprotein in theory. Overexpreasion of P-GP reduce the concentration of antiepileptic drugs in epileptic focus. This then caused drug fast. This view brings new hope for epilepsy treatment, such as using some inhibitors of P-gp to increace the antiepileptic drug concentration in brain.
