摘    要:
目的探讨转化生长因子-β1(TGF-β1)、基质金属蛋白酶-1(MMP-1)及其组织抑制因子-1(TIMP-1)在晚期血吸虫病(晚斑)肝组织和血清中的表达及其与肝组织病理学的关系。方法采用免疫组化sP法检测45例晚血(观察组)肝组织中TGF-β1、MMP-1、TIMP-1表达;采用EIASA法检测观察组和30例健康人(对照组)血清TIMP-1、TGF-β1、MMP-1水平。结果观察组肝组织中TGF-β1、TIMP-1表达强度与肝纤维化程度、有无合并HBV感染呈正相关(P均〈0.05);而MMP-1无相关性。与对照组比较,观察组血清TGF-β1、TIMP-1水平及TIMP-1/MMP-1值明显增高(P均〈0.01),并与肝纤维化程度、免疫组化表达强度呈正相关(P均〈0.01),而血清MMP-1差异无统计学意义。结论TGF-β1、MMP-1、TIMP-1与晚血肝纤维化的发生、发展密切相关,三者联合检测可作为晚血患者肝纤维化诊断和疗效评估指标。

Abstract:
Objective To explore the expression of TGF-β1 ,MMP-1 and TIMP-1 in the liver tissue of advanced schistosomiasis cases and its correlation with histopathology. Methods The liver tissues from 45 advanced schistosomiasis cases ( observed group) were biopsied and the expression of TGF-β1, MMP-1, TIMP-1 were detected by immunohistochemical method, while the level of which in the serum of observed group and 30 healthy people ( control group) were detected by ELISA method. Results The expression of TGF-β1 ,TIMP-1 in the liver tissues of observed group were positively related to the the degree of liver fibrosis and infection of HBV ( P 〈 0.05), but showed no statistical relation to MMP-1 ( P 〉 0.05 ) . The serum level of TGF-β1,TIMP-1 and TIMP-1/MMP-1 in the observed group were significantly higher than that in the control group(P 〈0.01). Conclusion TGF-β1,MMP-1 and TIMP-1 are closely involved in the formation of liver fibrosis in patients with advanced schistosomiasis. Their joint detection can be applied in the diagnosis of liver fibrosis and the evaluation of treatment effectiveness.
