阿加曲班联合依达拉奉治疗急性脑梗死的效果观察

目的 探讨阿加曲班联合依达拉奉治疗急性脑梗死(ACI)的效果.方法 回顾性分析2015年11月-2016年7月收治的90例ACI的临床资料,根据治疗方法分为依达拉奉组、阿加曲班组和联合用药组,每组30例.观察3组治疗效果、美国国立卫生院神经功能缺损量表(N IHSS)评分和Barthel指数,比较治疗前后血液流变学指标、凝血功能指标和血清超敏C反应蛋白(hs-CRP)、半胱氨酸(Hcy)水平.记录3组不良反应发生情况.结果 联合治疗组总有效率高于依达拉奉组和阿加曲班组,差异有统计学意义(P＜0.05).联合治疗组治疗后NIHSS评分、全血高切黏度、血浆黏度、红细胞压积和红细胞聚集、部分活化凝血酶原时间(APTT)、纤维蛋白原(FBG)、hs-CRP和Hcy低于依达拉奉组和阿加曲班组,而促凝血酶原时间(TT)、凝血酶原时间(PT)和Barthel指数高于依达拉奉组和阿加曲班组,差异有统计学意义(P＜0.05).3组均未出现严重不良反应.结论 联合应用阿加曲班和依达拉奉可有效提高ACI治疗效果,改善患者预后,提高生活质量,其疗效优于单独用药.

Effect of Argatroban Combined with Edaravone in Treatment of Patients with Acute Cerebral Infarction

Objective To investigate effect of Argatroban combined with Edaravone in treatment of patients with acute cerebral infarction (ACI).Methods Clinical data of 90 patients with acute cerebral infarction during November 2015 and July 2016 was retrospectively analyzed,and the patients were divided into Edaravone group and Argatroban group and Combined treatment group according to treatment method (n =30 for each group).In the three groups,therapeutic effects,national institute of health stroke scale (NIHSS) scores and Barthel index were observed,and levels of blood rheological indexes,indexes of blood clotting function,serum high sensitivity C-reactive protein (hs-CRP) and homocysteine (Hcy) before and after treatment were compared,and conditions of adverse reactions were recorded.Results The total effective rate in combined treatment group was significantly higher than those in Edaravone and Argatroban groups (P ＜ 0.05).After treatment,values of NIHSS score,whole blood high shear viscosity,plasma viscosity,hematocrit,erythrocyte aggregation,activated partial thromboplastin time (APTT),fibrinogen (FBG),hs-CRP and Hcy in combined treatment group were significantly lower,while values of thrombin time (TT),prothrombin time (PT) and Barthel index were significantly higher than those in Edaravone and Argatroban groups (P ＜ 0.05).There were no serious adverse reactions in the three groups.Conclusion Edaravone combined Argatroban can effectively improve therapeutic effect,improve prognosis and quality of life in treatment of ACI patients.The curative effect is significantly better than that by single drug.
