摘    要:目的:探讨椎动脉走行变异、扭曲的血流动力学改变及超声在其中的诊断价值.方法:回顾性分析27例椎动脉走行变异或椎动脉扭曲病人的超声图像特点,测定收缩期峰值流速(PSV)、阻力指数(RI),记录数据并进行分组比较,分析血流动力学改变规律.结果:27例病人中,16例椎动脉扭曲及11例椎动脉走行变异分别具有较为典型的超声表现,椎动脉扭曲组PSV显著高于对照组及对侧正常组(P<0.01),椎动脉走行变异组PSV显著高于对照组(P<0.01);椎动脉扭曲组、对侧正常组及椎动脉走行变异组RI均显著高于对照组(P<0.05).结论:椎动脉走行变异及椎动脉扭曲具有特征性声像图表现,超声诊断准确率高,并且能反映其血流动力学改变,可以此来预测血管管壁形成斑块的风险.

Hemodynamics abnormality of tortuosity and line variation vertebral artery
Abstract:Objective:To detect the hemodynamics of tortuosity of vertebral artery and congenital vertebral artery variation and value of the Color Doppler Ultrasonography in diagnosis. Methods:The ultrasonic image characteristics in 27 cases of tortuosity and line variation vertebral artery were retrospectively analyzed. The eak systolic flow velocity( PSV) and resistance index( RI) of tortuosity and line variation vertebral artery were measured with ultrasonoraphy to analyze its hemodynamics change patterns. Results:The characteristic ultrasonic image in 16 cases of tortuosity vertebral artery and 11 cases of line variation vertebral artery were found. The PSV in tortuosity vertebral artery group was significantly higher than that in normal control group and contralateral normal group( P<0. 01). The PSV in line variation vertebral artery group was significantly higher than that in normal control group(P<0. 01). The RI in tortuosity vertebral artery group,contralateral normal group and line variation vertebral artery group were significantly higher than that in normal control group(P<0. 05). Conclusions:The ultrasonic images in tortuosity and line variation vertebral artery are characteristic, the diagnostic accurate rate of ultrasonoraphy is high,which can reflect the change of hemodynamics,predict the risk of atherosclerosis plaque forming.
