五点定位法针刀治疗膝骨关节炎的临床研究

目的：观察采用五点定位法针刀治疗膝骨关节炎的临床疗效。方法：将符合诊断标准的60例患者随机分成对照组和治疗组，每组各30例，对照组采取经典定位法，治疗组采取五点定位法进行针刀治疗，对治疗前后的功能指标进行临床观察。结果：经过相关治疗后，两组患者的VAS评分、WOMAC评分、改良HSS评分均较治疗前与明显改善（p＜0.05，或p＜0.01），组间比较未见明显差异（p＞0.05）。中医证候评分，治疗组积分显示出更优越的短期效果（p＜0.05），疗程结束后1个月门诊随访，评分上两组之间没有明显区别。结论：五点定位法针刀治疗膝骨关节炎和经典定位法都具有良好的缓解疼痛、改善功能的效果，疗效相当，但五点定位法治疗适应面广，患者痛苦小，安全性更高，值得推广。

Clinical Effect of Needle Knife Based on the Five-point Positioning Method in Treatment of Knee Osteoarthritis

Objective: To observe the clinical efficacy of using the five-point localization method of needle knife in the treatment of knee osteoarthritis. Methods: Sixty patients who met the diagnostic criteria were randomly divided into a control group and a treatment group, 30 patients in each group. The control group adopted the classical positioning method and the treatment group adopted the five-point positioning method for needle knife treatment, and the functional indexes before and after treatment were clinically observed. Results: After the relevant treatment, the VAS score, WOMAC score and Modified HSS score of patients in both groups were significantly improved compared with those before treatment (p<0.05, or p<0.01), and no significant difference was seen between the groups (p>0.05). Chinese medicine evidence scores, the treatment group points showed superior short-term effects (p< 0.05), and there was no significant difference between the two groups in terms of scores at the 1-month outpatient follow-up after the course of treatment. Conclusion: Both the five-point localization method of needle knife for knee osteoarthritis and the classical localization method have good effects in relieving pain and improving function with comparable efficacy, but the five-point localization method has a wide range of treatment indications, is less painful for patients, and is safer and worth promoting.
