胃管及开塞露联合生理盐水深部保留灌肠治疗老年便秘的疗效观察

目的观察胃管及开塞露联合生理盐水深部保留灌肠治疗老年便秘的临床疗效。方法将87例老年便秘患者随机分为实验A组（n=29）、对照B组（n=29）、对照C组（n=29），实验A组采用胃管与一次输液器相连灌肠，灌肠液为60mL开塞露加250mL生理盐水的稀释液，灌肠深度为25～30cm；对照B组采用一次性肠道冲洗器，灌肠液为60mL开塞露加250mL生理盐水的稀释液，灌肠深度为7～10cm；对照C组采用胃管与50mL注射器，灌肠液为开塞露60mL，灌肠深度为25～30cm。观察三组的灌肠效果、灌肠液外溢情况、胃肠痉挛性疼痛、里急后重等并发症发生情况。结果三组疗效比较，实验A组总有效率明显优于对照B组、对照C组（P〈0．01）。灌肠液外溢情况显示，实验A组明显少于对照B组（P〈0．01）。胃肠痉挛及里急后重感等并发症发生情况显示，实验A组明显少于对照B、C组（P〈0．01）。结论应用胃管及开塞露联合生理盐水深部保留灌肠治疗老年便秘，提高了灌肠效果，减轻了患者的痛苦。

Observation on effect of deep retention enema with nasogastric tube and Glycerine combined with saline in the treatment of senile constipation

Objective To observe the effect of gastric tube and Glycerine combined with 0.9% NS deep retention enema in the treatment of senile constipation clinical.Methods 87 senile constipation patients were randomly divided into experimental group A(n = 29),control group B(n = 29),control group C(n = 29).The experimental group A was given an enema in depth of 25-30 cm with 60 mL Glycerine plus 250 mL 0.9% NS dilution through gastric tube and a transfusion device connected;control group B was given disposable intestinal flushing and an enema with 60 mL Glycerine and 250 mL 0.9% NS dilution in depth of 7-10 cm;control group C was given gastric tube and 50 mL syringe and an enema with 60 mL Glycerine in depth of 25-30 cm.The effect of the enema,externalities,gastrointestinal spasm pain,tenesmus and other complications of the three groups were observed.Results Compared with the effects of the three groups,the total effective rate of experimental group A was superior to control group B and control group C(P < 0.01).Enema liquid overflow condition of experimental group A was obviously less than control group B(P < 0.01),gastrointestinal spasm and tenesmus feeling and other complications in experimental group A was significantly less than control group B and C(P < 0.01).Conclusion The application of gastric tube and Glycerine combined with 0.9% NS deep retention enema in the treatment of senile constipation can improve the enema effect and reduce the pain of patients.
