2型糖尿病患者血浆同型半胱氨酸水平与合并血管病变的关系

目的探讨血浆同型半胱氨酸（Hcy）水平对2型糖尿病合并血管病变的相关性。方法对泸州医学院附属医院收治的100例2型糖尿病患者以及80例健康体检老人进行研究,并根据患者Hcy水平将糖尿病患者分为高Hcy组以及正常Hcy组,并对2型糖尿病患者进行生化指标以及并发症的比较,同时对颈动脉内膜中层厚度（IMT）增厚组以及IMT正常组,健康对照组Hcy水平、高同型半胱氨酸血症的发生情况进行比较。结果两组患者肾小球滤过率、肌酐含量、尿酸含量、尿白蛋白排泄率以及糖化血红蛋白含量、IMT增厚程度比较,差异均有统计学意义（P〈0.05）;IMT增厚组患者其Hcy水平以及高同型半胱氨酸血症患者百分率比较,差异有统计学意义（P〈0.05）,而正常对照组和IMT正常组比较差异有统计学意义（P〈0.05）;高Hcy组其并发症发生概率明显高于正常Hcy组（P〈0.05）。结论血浆Hcy水平与2型糖尿病患者合并出现血管病变有明显的相关性,当其含量增加时,会导致患者血管病变的发生概率明显增加。

Plasma Hcy level in type 2 diabetes mellitus and the correlation of vascular lesions

Objective To discuss the correlation of hematoplasma Hcy level with T2DM combined with vascular lesion.Methods 100 patients with T2DM in the affiliated hospital of Luzhou medical college and 80 healthy medical examination old people were studied and the DM patients were divide into high Hcy level group and normal Hcy level group,and the biochemical indicator and complication among these T2DM patients were taken a comparison.Meanwhile,the Hcy level and development of Hyperhomocysteinemia of IMT thickening group with that of IMT normal group and healthy control group were also compared.Results The findings of the contrast shows statistically significant difference in age,course of hypertensive disease,GFR,Crea-content,UA-content,urinary albumin excretion rate,glycosylated hemoglobin-content and the degree of IMT thickening of the two groups(P < 0.05);there was still statistically significant difference in the comparison of Hcy level and the percentage of Hyperhomocysteinemia patients among the IMT thickening group(P < 0.05);the normal control group and IMT normal group also indicated statistically significant difference(P < 0.05);the incidence of complications of high Hcy level group was dramatically higher than that of normal level group(P < 0.05).Conclusion There is an obvious correlation between hematoplasma Hcy level and T2DM combined with vascular lesion,a upgrade of Hcy level leads to an evident increase of incidence of vascular lesion.
