内皮细胞蛋白C受体在狼疮性肾炎患者血浆中的表达

目的分析可溶性内皮细胞蛋白C受体（sEPCR）在狼疮性肾炎（LN）患者血浆中的表达,探讨其与LN临床及病理类型的关系。方法回顾性分析我院2009年1月～2010年10月肾内科及风湿免疫科收治住院的LN患者54例肾脏病理资料（观察组）,另选择我院同期健康体检者54例作为对照组,两组患者均采用ELISA法检测血浆中sEPCR水平,并进行相关分析统计。结果①观察组血浆sEPCR及sEPCR mRNA水平高于对照组（P〈0．01）。Ⅳ型LN患者sEPCR水平高于Ⅲ型和Ⅴ型（P〈0．05）。②LN患者sEPCR与SLEDAI、24h蛋白尿水平和CRP均呈正相关（P〈0.01或P〈0．05）。结论LN存在血管内皮细胞损伤,血浆sEPCR水平可作为判断LN疾病活动性及肾脏损伤程度的指标。

Clinical significance of plasma soluble endothelial protein C receptor level in patients with lupus nephritis

Objective To study the plasma level of soluble endothelial protein C receptor(sEPCR) in patients with lupus nephritis(LN) and analyze its association with clinical manifestation and pathological classification of LN.Methods The kidney pathological materials of 54 patents with LN(observation group) admitted to renal and rheumatism department of our hospital from January 2009 to October 2010 were analyzed retrospectively,another 54 healthy persons in the same period were chosen as control group.The patients of the two groups all used ELISA to test plasma sEPCR level,and took relative analysis.Results ①The plasma level of sEPCR and sEPCR mRNA in observation group were higher than those in control group(P < 0.01).The sEPCR level in type Ⅳ LN patients was higher than those in type Ⅲ and Ⅴ(P < 0.05).②The sEPCR level in patients with LN was positively correlated with SLEDAI,24 hours urine protein and CRP(P < 0.01 or P < 0.05).Conclusion The endothelial injury may play an important role in the pathogenesis of LN.The plasma level of sEPCR can reflect the disease activity and the severity of renal injury.
