布托啡诺用于剖宫产术后镇痛临床研究

目的探讨布托啡诺在剖宫产术后患者自控静脉镇痛（PCIA）中的应用效果。方法选择我院2010年10月～2011年12月行剖宫产术的产妇260例,将其随机分为观察组与对照组,每组各130例。观察组产妇给予布托啡诺进行PCIA,对照组患者给予芬太尼进行PCIA。在剖宫产术后4、8、12、24及36 h分别记录患者的疼痛评分（采用疼痛视觉模拟评分法,VAS）与镇静评分（采用Ramsay法）,同时观察患者术后并发症的发生情况及患者满意度情况。结果观察组与对照组患者在术后4、8、12、24及36 h各时间点疼痛评分比较,差异无统计学意义（P〉0.05）,但观察组患者术后各时间点镇静评分明显高于对照组,差异有统计学意义（P〈0.05）。两组患者术后生命体征均较为稳定,无呼吸抑制现象发生。观察组患者满意度明显高于对照组,差异有统计学意义（P〈0.05）。结论布托啡诺用于剖宫产术后PCIA镇痛效果可靠、镇静适度、不良反应发生率低,具有重要的临床应用价值。

Clinical study of Butorphanol in the postoperative analgesia of caesarean section

Objective To evaluate the application effect of controlled intravenous analgesia(PCIA) of Butorphanol in the postoperative analgesia of caesarean section.Methods 260 patients of epidural anesthesia were divided into observation group and control group,130 patients in each group.The observation group used Butorphanol,and the control group used Fentanyl.The patients were observed at postoperative 4,8,12,24,36 hours respectively.Visual ache simulation score(VAS) and Ramsay were used to record the pain and sedation.The side effects and patient’s satisfaction were observed.Results There was no difference between the observation group and the control group in visual ache simulation score at 4,8,12,34,35 hours(P > 0.05).In the observation group at each time point after operation sedation score significantly higher than that of the control group,the difference was statistically significant(P < 0.05).In two groups of patients with postoperative vital signs were stable,and there was no respiratory depression phenomenon.The observation group patient’s satisfaction was significantly higher than that of the control group,the difference was statistically significant(P < 0.05).Conclusion The effect of epidural Butorphanol for postoperative analgesia is very good,and the incidences of complications keep in little.
