助产一对一全程陪伴在高龄产妇分娩中的应用效果观察

目的探讨高龄产妇实施助产一对一全程陪伴的效果。方法选择2009年6月～2010年6月在我院住院分娩的高龄产妇120例,按随机数字表法将其分为观察组和对照组,每组各60例。对照组按传统助产模式分娩,观察组实施助产一对一全程陪伴。比较两组心理状态、分娩方式、产后出血和新生儿情况。结果观察组产妇的SAS评分、焦虑发生率及产后出血发生率和产后出血量均显著低于对照组（P〈0.01）,自然分娩例数明显多于对照组（P〈0.01）,而剖宫产例数明显少于对照组（P〈0.01）,自然分娩产妇的第一产程、第二产程及总产程时间均显著短于对照组（P〈0.05）,新生儿正常率明显高于对照组（P〈0.01）。结论助产一对一全程陪伴能有效减轻高龄产妇焦虑情绪,提高自然分娩率,缩短产程,减少产后出血和新生儿窒息,值得在高龄产妇中推广应用。

Application effect observation of one-to-one midwifery accompany in the delivery of elderly parturient women

Objective To discuss the efficacy on one-to-one midwifery accompany in the delivery of elderly parturient women.Methods 120 cases of elderly parturient women giving birth in our hospital from June 2009 to June 2010 were randomly divided into observation group and control group by random number table,each group had 60 cases.The control group gave birth by traditional midwifery model and the observation group gave birth by the implementation of one-to-one midwifery accompany.Mental status,mode of delivery,postpartum hemorrhage and newborn conditions of both groups were compared.Results SAS score,incidence of anxiety and amount and incidence of postpartum hemorrhage were significantly lower in the observation group than that in the control group(P < 0.01).Natural childbirth was significantly more and cesarean section significantly less in the observation group than that in the control group(P < 0.01).The first stage and second stage of labor and total labor duration were significantly shorter in the observation group than that in the control group(P < 0.05) and neonatal normal rate was also significantly higher in the observation group(P < 0.01).Conclusion One-to-one midwifery accompany can effectively reduce the anxiety of older mothers,improve the natural birth rate,shorten the birth process,reduce postpartum hemorrhage and reduce incidence of newborn asphyxia,thus it is worthy of clinical application in the delivery of older mothers.
