PVP与PKP治疗椎体压缩性骨折140例的疗效及对并发症的影响

目的：探讨PVP和PKP治疗椎体压缩性骨折的效果及对并发症的影响。方法：选取2012年9月—2014年12月期间我院确诊治疗的椎体压缩性骨折患者140例，随机分为PKP组和PVP组各70例。PVP组患者给予常规经皮椎体成形术治疗，PKP组患者给予经皮球囊扩张椎体后凸成形术治疗。随访12个月，统计分析所有患者治疗前后Cobb角、椎体前缘高度的变化；VAS法评估治疗前、治疗后2、4、6个月的疼痛情况；记录发生的并发症。结果：PKP组患者治疗后Cobb角、椎体前缘高度均明显优于PVP组（P<0.05）；PKP组患者治疗后2、4、6个月的VAS得分和并发症发生率均明显低于PVP组（P<0.05）结论：经皮球囊扩张椎体后凸成形术治疗可更有效地改善椎体压缩性骨折患者Cobb角、椎体前缘高度，缓解患者的疼痛，且可有效减少并发症的发生。

Curative effect of PVP and PKP in the treatment of 140 cases of patients with vertebral compression fractures and their complications

Objective:To discuss the curative effect and complications of PVP and PKP in the treatment of patients with vertebral compression fractures. Methods:One hundred and forty patients with vertebral compression fractures were selected and randomly divided into PKP group and PVP group with 70 cases each. PVP group was treated with conventional percutaneous vertebroplasty while PKP group with percutaneous balloon kyphoplasty and they were all followed up for 12 months. The changes of Cobb angle and the anterior height of vertebral body were statistically analyzed before and after treatment and pain conditions before treatment and 2, 4 and 6 months after were assessed by visual analogue scale (VAS), and the complications were also recorded. Results:The changes of Cobb angle and the anterior height of vertebral body, and the VAS scores at postoperative 2, 4 and 6 months and the incidence of complications were signiifcantly better or lower in the PKP group than in the PVP group (P<0.05). Conclusion:Percutaneous balloon kyphoplasty can more effectively improve Cobb angle and the anterior height of vertebral body, relieve the pain and reduce the incidence of complications in the treatment of patients with vertebral compression fractures.
