比例辅助通气模式在新生儿机械通气中的应用观察

目的了解比例辅助通气在新生儿机械通气中的应用效果。方法对15例需要机械通气的新生儿,首先进行常规机械通气（SIMV模式）,分别于通气1小时后改为SIMV＋PAV模式通气1小时,观察人机对抗次数、每分钟通气量（MV）、动态肺顺应性（Cdyn）、血压、心率和呼吸频率的变化。结果所有病例的人机对抗次数明显减少,而MV、Cdyn的数值均呈明显增加,三个指标数值变化的差异具有统计学意义;血压、心率和呼吸频率的数值变化的差异无统计学意义。结论PAV＋SIMV模式,明显减少了人机对抗,提高了新生儿通气的舒适水平,减少了患儿所需的呼吸功。

Observation of proportional assist ventilation application in 15 cases of neonatal mechanical ventilation

Objective To understand the effect of proportional assist ventilation＇s application in the neonatal mechanical ventilation.Methods For the 15cases of newborn who need for mechanical ventilation, first of all use conventional mechanical ventilation （SIMV mode）, in 1 hour later change to the ventilation SIMV ＋ PAV mode ventilation for 1 hour, to observe the number of man-ventilator mismatch, minute ventilation （MV）, Dynamic lung compliance （Cdyn）, blood pressure, heart rate and respiratory rate.Results The number of man-ventilator mismatch of all cases reduced obviously, and the scales of MV, Cdyn showed a marked increase, the difference of the three indicators have statistically significant; Blood pressure, heart rate and respiratory rate changes was not statistically significant difference. Conclusions PAV ＋ SIMV mode, significantly reduce the man-ventilator mismatch, and increase the comfort level of neonatal ventilation, decrease the necessary breathing work of sick children.
