开放性胫腓骨骨折不同治疗方式效果观察

目的：采用不同手术方法治疗开放性胫腓骨骨折，探讨其优缺点及适应证。方法：166例开放性胫腓骨骨折患者分别采用加压钢板、外固定支架及交锁髓内钉三种不同的固定方法治疗，对比分析其疗效。结果：三种治疗法在疗效（离床下地时间，骨折愈合时间，骨折不愈合发生情况及伤口感染情况）上有显著性差异，加压钢板离床下地活动时间和骨折愈合时间最长，伤口感染、骨不连发生率最高。结论：三种治疗方式各有特点，应根据具体情况选用，以达到最好治疗效果。

Operative effects of open tibiofibular fractures

Objective:To investigate the effects of different operative method in treating open tibiofibular fractures.Methods:One hundred and sixty-six cases which were stabilized by three different management:steel plate,external fixation and interlocking nail respectively were analyzed retrospectively.Results:Three methods had significant difference(P<0.05)on four aspects:the time of early active moment,the time of fracture recovery,nonunion and wound infection.The method of internal fixated with steel plate had the longest time of early active moment and fracture recovery,the highest rate of wound infection and nonunion.Conclusion:Interlocking nail has the characteristics of stability,few complications and high rate of fracture recovery,which is the most suitable treatment of the three methods,especially for many segment fractures.External fixation is simple and it has the characteristics of few injury,bad stability and it is suit to treat seriously open fractures.Steel plate is stable, and It is a good choice for nearby joint fractures ,but high rate of infection and nonunion must to be considered.
