中西医结合治疗支气管哮喘缓解期30例疗效观察

目的观察补肺益肾中药联合西药对支气管哮喘缓解期患者肺功能及炎性因子的影响。方法将60例支气管哮喘缓解期患者随机分为2组,对照组30例予孟鲁斯特、沙美特罗/丙酸氟替卡松治疗,疗程为3个月;治疗组30例在对照组治疗基础上加补肺益肾中药治疗,14 d为1个疗程,治疗3个疗程。观察2组临床疗效,2组治疗前、治疗后12周进行肺功能测定、血嗜酸性粒细胞计数测定及细胞因子测定。结果治疗组总有效率96.67%,对照组总有效率80.00%,2组总有效率比较差异有统计学意义(P<0.05),治疗组疗效优于对照组。2组治疗后第1 s用力呼气容积(FEV1)、白细胞介素12(IL-12)较本组治疗前增加(P<0.05),呼气流量峰值(PEF)昼夜波动率、血嗜酸性粒细胞计数及白细胞介素4(IL-4)均较本组治疗前降低(P<0.05),且治疗组改善明显(P<0.05)。结论补肺益肾中药联合西药能明显改善支气管哮喘缓解期患者肺功能,调整细胞因子水平。

Observation of bronchial asthma at remission treated by the combined traditional Chinese medicine and western medicine treatment

Objective To investigate the effect of the combined traditional Chinese medicine and western medicine treatment on lung function and inflammatory factors in patients with bronchial asthma at remission. Methods 60 patients with bronchial asthma in remission stage were randomly divided into two groups. Control group （ n = 30） was given Montelukast, salmeterol/fluticasone propionate therapy, treatment for 3 months. Treatment group was given Chinese medicine for strengthening lung and nourishing kidney on the basis of conventional therapy. The course was 14 days. 3 courses were carried on. The clinical efficacy was observed. Lung function, eosinophil count and cy- tokines were measured before and 12 weeks after treatment. Results The total effective rate in treatment group （96.67%） was higher than that in control group （80.00%, P 〈 0.05 ）. FEV1 and IL - 12 after treatment was in- creased as compared with that before treatment in two groups （ P 〈 0.05 ）. Peak expiratory flow （PEF） circadian vol- atility, eosinophil count and IL - 4 after treatment was decreased as compared with that before treatment in two groups （ P 〈 0.05 ）. The improvement in treatment group was superior to that in control group （ P 〈 0.05 ）. Conclusion Chinese medicine for strengthening lung and nourishing kidney combined with western medicine can significantly im- prove lung function of bronchial asthma in remission, adjust the levels of cytokines.
