摘    要:
目的研究影响2型糖尿病患者血清同型半胱氨酸(Hcy)水平变化的相关因素。方法随机选择280例患者,分组为正常血糖组(Ⅰ组)98例,糖耐量降低组(Ⅱ组)66例,2型糖尿病组(Ⅲ组)116例。所有受试对象均测量体质量、身高、血压、血糖、血脂、Hcy、肌酐及胰岛素水平。结果Ⅲ组和Ⅱ组的Hcy高于Ⅰ组,差异有统计学意义(P<0.05)。多元线性回归分析表明,Hcy与胰岛素抵抗(IR)、肌酐、糖化血红蛋白(HbA1c)呈正相关(r=0.479,P=0.036;r=0.451,P=0.023;r=0.432,P=0.019);男性Hcy水平高于女性(r=-0.378,P=0.021)。结论2型糖尿病患者血清Hcy水平高低受IR、肌酐水平、糖化血红蛋白水平、性别等因素影响。

Abstract:
Objective To study the correlation facts for change of serum homocysteine(Hcy) in patients with type 2 diabetes mellitus (DM). Method 280 patients were randomly divided into 3 groups, including normal group ( Ⅰ group, 98 cases), impaired glucose tolerance group( Ⅱ group, 66cases), type 2 DM group (Ⅲ group, 116 cases). All of them were tested which involved in height, weight, blood pressure, plasma Hey, lipids, blood glucose, insulin, serum creatinine and HbAlc. Result The levels of Hcy were increased significantly in Ⅲ and Ⅱ groups, compared with Ⅰ group (P〈0.05). Multiple linear regression analysis showed that Hcy was positively correlated with IR, serum creatinine and HbAlc (r=0.479, P=0.036; r=0.451, P=-0.023; r=0.432, P=-0.019). The serum Hcy could be influenced by gender. Male had more risk for high level of Hcy(r=-0.378,P=0.021). Conclusion The level of serum Hcy could be influenced by creatinine, HbAlc, IR and gender in in patients with type 2 diabetes mellitus.
