Caspase-3蛋白在喉鳞癌组织中表达及与喉癌细胞凋亡的对比研究

目的探讨半胱氨酸天冬氨酸蛋白酶3(Caspase-3)蛋白在喉鳞癌组织中的表达以及与喉癌细胞凋亡的对比关系。方法应用免疫组化SP法分别检测31例喉鳞癌组织和15例声带息肉组织中Caspase-3蛋白表达,用末端脱氧核苷酸转移酶介导的荧光素末端标记法(TUNEL)检测喉鳞癌细胞凋亡率,计算凋亡指数(AI)值并与Caspase-3喉癌组织中表达阳性率结果对比。结果 Caspase-3在喉鳞癌组织中的表达低于声带息肉组织(P<0.01)。Caspase-3的表达在喉癌组织分化程度、有无淋巴结转移及临床分期的差异具有统计学意义(P<0.05),在临床分型、有无吸烟、性别及年龄方面的差异无统计学意义(P>0.05)。喉癌组中凋亡的表达凋亡指数明显低于癌旁组,在统计学上有显著性差异(P<0.05)。喉鳞癌组织中Caspase-3的表达与TUNEL阳性细胞(凋亡细胞)数呈显著正相关,差异具有统计学意义(r=0.790,P<0.0)。结论 Caspase-3蛋白在喉鳞癌中低表达可促进喉鳞癌的发生、发展,其蛋白检测可作为判断喉癌分化、浸润转移及临床分期的重要指标。

Study on expression of Caspase-3 in laryngeal carcinoma and its correlation with apoptosis

Objective To explore the expression of Caspase-3 and its correlation with apoptosis of cells in laryngeal squamous cell carcinoma.Methods A total of 31 samples of laryngeal carcinoma tissue and 15 samples of polyps of vocal cord tissue were analyzed for the expression of Caspase-3 by immunohistochemical method and AI（apoptosis index） by terminal-deoxynucleotidyl transferase mediated nick end labeling（TUNEL）,and then compared with the expression of Caspase-3.Results The expression of Caspase-3 in laryngeal squamous cell carcinoma was significantly higher than that in polyp of vocal cord tissue（P0.01）.In laryngeal carcinoma,the expression of Caspase-3 was associated with differentiation of tissue,metastasis in lymph glands and clinical stages（P0.05）,but it was independent of clinical classification,smoking history,gender and age（P0.05）.The significance of AI in laryngeal squamous cell carcinoma was significantly lower than that in polyp of vocal cord tissue（P0.05）.A significantly positive relationship had been found between Caspase-3 and AI in laryngeal squamous cell carcinoma（P0.05）.Conclusion The low expression of Caspase-3 is helpful in diagnosis of laryngeal squamous cell carcinoma and Caspase-3 can be used as an important index for its differentiation,invasion and clinical stage.
