白内障晶体肿胀期并发青光眼早期手术的临床观察

摘    要:
目的:探讨白内障晶体肿胀期并发急性闭角性青光眼行白内障摘除术的临床效果。方法:对我院2006年6月-2008年6月共收治的34例(50眼)白内障晶体肿胀期并发急性闭角性青光眼行白内障摘除术的患者,采用超声乳化白内障摘除术联合人工晶体植入术,观察术后视力及并发症等。结果:34例(50眼)中,术后所有患者视力均有不同程度的提高,眼压得到控制,术后1周内,40眼眼压在正常范围。有10眼发生高眼压,经药物治疗1月,停药后6眼眼压恢复正常,4眼仍有高眼压,行青光眼滤过术,眼压均控制正常。结论:晶体肿胀期并发急性闭角性青光眼,药物控制眼压后早期行超声乳化白内障摘除联合人工晶体植入术,可明显减少视力损害,挽救残存视力。高眼压时间长,房角粘连范围广则应一期行青光眼白内障联合手术。

Abstract:
Objective: To explore clinical effect of the cataract lens swelling superinduce acute angle closure glaucoma cataract surgery. Method: 34 cases (50eyes) with cataract lens swelling period of acute angle closure glaucoma were operated by the surgery using phacoemulsification cataract and implanting artificial lens. pest-operative visual acuity and complications were observed. Resuit: All the patients experienced varying improvement in their sights with controlled intraocular pressure, 40 cases range within normal level, the high intraocular pressure of the 10 eyes returned to normal after 1 month of drug therapy, the high intraocular pressure of the 4 eyes come under control after trabeculectomy. Conclusion: The damage of acute angle closure glaucoma in lens swelling period to the sight can be greatly reduced by the phacoemulsification intraocular lens early after the drug therapy keeping the surving sight. Those who suffer from high intraocular pressure for a long time and fargoing goniosynechia are supposed to have beth the trabeculectomy surgery and phacoemulsification intraocular lens at the one-stage.
