晶状体超声乳化术中对瞳孔不能散大者的处理

目的探讨白内障施行晶状体超声乳化时对瞳孔不能散大者的处理措施。方法24例(24眼)瞳孔不能散大的白内障术中经巩膜或角膜隧道切口进入前房,以黏弹剂及黏弹剂弯针头分离虹膜或以囊剪行瞳孔缘剪开,瞳孔散大至5~6mm,环形撕囊或截囊,行晶状体超声乳化囊袋内植入人工晶状体,术后观察视力、瞳孔及眼部反应。结果经术中处理后瞳孔可散大至5~6 mm,术后瞳孔恢复圆形,视力均≥0.5,眼部无异常反应。结论白内障合并瞳孔不能散大者经术中处理瞳孔,可顺利实施超声乳化和人工晶状体植入术,术后获得良好视力和圆瞳孔,无并发症。

Treatment of small pupil in phacoemulsification

Objective To investigate the treatment of small pupil in cataract phacoemulsification.Methods 24 eyes of 24 patients were operated using scleral tunnel or clear corneal incision.The posterior synechia was dissected by viscoelastic substances and it's needle,or making small radial cuts at pupilary margin.The pupil was dilated to 5-6 mm,then circular capsularhexis or capsulotomy phacoemulsification and posterior chamber intraocular lens implantation was done.The changes of vision,pupil and ocular reaction were observed.Results Pupils could be dilated to 5-6 mm after these treatment.Postoperative vision was 0.5 or better than it and the pupils kept round.No severe complications happened.Conclusion Through these methods,small pupils could be dilated big enough to perform phacoemulsification and intraocular lens implantation.
