胃癌术前活检及手术切除标本中MMP—9的表达和意义

目的 探讨基质金属蛋白酶MMP-9在胃癌组织中的表达，及其与胃癌临床病理特征的相关性。方法 采用免疫组织化学S-P法检测10例术前胃镜活检胃癌组织标本和40例胃癌手术切除的组织标本中MMP-9的表达情况。结果 40例胃癌组织标本中MMP-9阳性表达29例（72.5%），胃癌组织中MMP-9表达增高与胃癌低分化类型及淋巴结转移相关。10例胃镜活检标本中阳性表达4例（40.0%），相应的手术切除组织标本有7例阳性表达，活检组织阳性表达灵敏度为57.1%。结论 MMP-9高表达提示胃癌的恶性生物学行为，术前胃镜活检组织及胃癌手术切除组织标本MMP-9表达的联合检测，对临床胃癌治疗方案的选择、预后的评估有一定参考价值。

Expression and Significance of Metalloproteinase-9 in Human Primary Gastric Carcinoma

Objective To study the expression of matrix metalloproteinase-9(MMP-9)in human gastric carcinoma,analyse the correlation between its expression and the clinicopathologic features of gastric carcinoma.Methods 40 cases of specimen from gastric carcinoma resection as well as 10 from preoperative gastroscopic biopsies were investigated by immunohistochemcal S-P method with monoclonal antibody against MMP-9.Results Among 40 cases,29 cases were MMP-9 positive(72.5%).The overexpression of MMP-9 was correlated to lymph node metastasis and poorer differentiation(P<0.01 and P<0.05,respectively).4 out of the 10 biopsies and 7 out of the 10 corresponding resected specimens were MMP-9 positive(40.0% and 70.0%,respectively).The sensitivity of MMP-9 positive expression in biopsy was 57.1%.Conclusion Our finding suggest that the overexpression of MMP-9 may indicate the more malignant biological behavior of gastric carcinoma.In addition,co-detection of MMP-9 in preoperative biopsy and postoperative gastric cancer tissue may prove to be valuable to clinic for selecting suitable therapy and assessing prognosis.
