BC-5500白细胞警示信息与手工镜检结果的对比分析

目的探讨BC-5500五分类血液细胞分析仪提示的白细胞警示信息与手工镜检结果对比分析。方法将连续20d提示有白细胞警示信息的158例标本住院患者,同时用传统的手工法制成薄厚适宜的血液涂片,经瑞氏-姬姆萨染色后进行手工白细胞分类,结果与仪器白细胞分类结果相比较。结果158例白细胞警示信息主要有核左移、未成熟细胞、异型或异常淋巴细胞、白细胞总数及五类白细胞数的升高或降低,白细胞分类散点图（DIFF）灰色散点图。手工分类符合率分别为45．7％、29．6％、25．0％、96．1％、90.9％。其中11例DIFF灰色散点图,仪器未能进行白细胞分类,手工镜检发现幼稚血细胞10例,结合临床诊断基本为白血病细胞;另147例白细胞警示信息标本BC5500白细胞分类与手工镜检白细胞分类结果进行相关性对比分析,中性粒细胞（r）0．9970、淋巴细胞（r）0．9299、单核细胞（r）0．9627、嗜酸性粒细胞（r）0．9972、嗜碱性粒细胞（r）0．8592。结论对于白细胞警示信息中白细胞总数及各分类细胞数与手工镜检相关性良好,但是DIFF灰色散点图,白细胞未能分类标本必须手工镜检复查。因为BC5500无法检测各类幼稚细胞的性质及质量,无法分别出中性粒细胞杆状核及分叶核。

The contrastive analysis on BC-5500 white blood cell warning information and manual microscopic exam result

Objective To investigate and discuss the eomparision of the leucocyte warning information signalled by BC-5500 five classification blood cell analysis instrument and by the method of handmade mirror inspecting. Methods To detect 158 samples isolated from the hospitalized patients with the leucocyte warning information of white blood cells indicated by the way of the traditional handmade legal institutions to classfy the leucocyte with Mrs. omengirl Msa dyeing queen and compare the results for 20 days tuning. Results The main information con- tained in the 158 cases of warning white cells are left slip of the core, abnormal lympheells and the increase or reduce of the total sums of white cells and the five kinds of leucocytes count, and the spreading point diagram is abnormal. The coincidence rate of classification by hand is 45.7% , 29.6% , 25.0% , 96.1% , and 90.9% respectively. Abnormal person of 11 examples of leucocytes among them scattered point diagram, the instrument failed in carting out leucocyte classification , and 10 samples of childish blood cells were detected by handmade mirror , which are basically leucocythemia cells combined with the clinical diagnosis. The correlativity of the results of the other 147 examples of leucocyte warning information specimen BC-5500 leucocyte classifies and the ones by handmade mirror inspection were compared, with the neuter granuloeyte （r） 0. 9970, B lymphocyte B （r） 0. 9299, the cell （r） only 0. 9627, acid granulocyte （r） 0. 9972, and basophil （r） 0. 8592. Conclusion The correlativity of the leucocyte sum and every classification cell numbers of the leucocyte in warning information and the one with handmade mirror is fine, but leu- cocyte scattered point diagram failed in elassifing extraordinarily specimen, so it is obliged to be rechecked by means of handmade mirror, because BC-5500 has no way to check the character and mass of all kinds of children cells, and also couldn＇t identify the neuter granulocyte rhabditiform core and the leaf core.
