第二代椎间盘镜治疗腰椎间盘突出症的中期疗效观察

目的观察第二代椎间盘镜手术系统治疗腰椎间盘突出症的中期临床疗效。方法回顾分析2004年3月至2005年12月间64例腰椎间盘突出症患者采用第二代椎间盘镜手术系统治疗的中期临床疗效。结果本组64例,随访51～72个月,平均60个月。按Nakai评价标准评定,优48例,良13例,可3例,差0例,优良率95.31%。手术时间40～120 min,平均68 min。术中失血15～40 mL,术中硬脊膜损伤1例,神经损伤1例,无椎间隙感染及术后复发病例。结论第二代椎间盘镜微创手术系统治疗腰椎间盘突出症是一种技术先进、组织创伤少、对脊椎稳定性破坏小、临床治疗效果良好的理想手术方法。

Observation on Mid-term Effect of Treatment of Iumbar Disc Herniation by MicroEndoscropic Discectomy

Objective To observe the clinical on mid-term effect of treatment of lumbar disc herniation by microendoscropic discectomy.Methods Review and analyz on mid-term the clinical effects of 64 cases of lumbar disc herniation by microendoscopic discectomy from March 2004 to December 2005.Results According to Nakai standard,48 out of 64 cases are rated as excellent,13 cases are good,3 cases are fine,none of them is poor,the rate of excellent and good occupies 95.31% during follow-up 51～72 months（60 months on average）.The operations lasted from 40 to 120 min with 68 minutes on average,blood losing of each is 15～40 mL.One of them suffered spinal dura mater during the operation and another one was hurt in nerve,no intervertebral space infection and recidivation after the operation.Conclusion The microendoscopic discectomy operational system treating the lumbar disc herniation is an ideal and effective oprating method in treating the lumbar disc herniation,it is a kind of advanced techique with less trauma to the tissus and lees hurt to the stability to the spinal vertebrae.
