多西紫杉醇联合顺铂治疗复发性小细胞肺癌临床观察

为了观察多西紫杉醇联合顺铂治疗复发或进展小细胞肺癌(SCLC)的疗效和安全性,应用多西紫杉醇联合顺铂治疗40例复发或进展SCLC,多西紫杉醇75 mg/m2,静脉滴入,d1,顺铂30 mg/m2,静脉滴入,d1～d3,21 d为1个周期.40例均可评价疗效,完全缓解率为10.0％(4/40),部分缓解率为52.5％(21/40),稳定率为27.5％(11/40),进展率为10.0％(4/40),总有效率为62.5％(25/40),中位疾病进展时间为21周,中位生存时间为30周,1年生存率为12.5％(5/40).主要毒副反应为血液学毒性,Ⅲ～Ⅳ度白细胞减少率为25.0％(10/40),无粒细胞减少性发热,Ⅲ度血小板下降率为2.5％(1/40),恶心呕吐等胃肠道反应发生率为75.0％,无严重肝、肾及神经毒性,全组无毒性相关死亡病例.初步研究结果提示,多西紫杉醇联合顺铂作为二线方案治疗复发或进展小细胞肺癌有较好的疗效,不良反应可以耐受.

Combined chemotherapy of taxotere and cisplatin in the treatment of relapsed small cell lung cancer

The objective of this study was to evaluate the efficacy and safety of the taxotere combination with cisplatin regimen in patients with relapsed small cell lung cancer.A total of forty patients with a pathologic diagnosis enrolled in the study.They were treated with taxotere 75 mg/m2 infusion for 1 h on day 1,and cispaltin 30 mg/m2 infusion on day 1 to 3.It was repeated every 21 days.The overall response rate was 62.5% with 10.0% of complete remission,and 52.5% of partial remission 27.5% of stability rate,10.0% of progress rate.The median time to progression was 21 weeks,the median survival time was 30 weeks,and the probability of 1-year survival was 12.5%.The main toxicities were hematological toxicities,nausea and vomiting.Grade Ⅲ to Ⅳ leukopenia was occurred in 25.0% of patients,and grade Ⅲ thrombocytopenia was occurred in 2.5% of patients.There were no febrile neutropenia.Nausea and vomiting were occurred in 75.0% of patients.There were no neurotoxicity,liver and kidney toxicity,and treatment-related death.In conclusion,the taxotere combination with cisplatin regimen shows a better chemotherapeutic effect and less toxicity.
