联合检测糖化血红蛋白与尿微量白蛋白对糖尿病肾病的临床意义

摘    要:目的探讨联合检测糖化血红蛋白与尿微量白蛋白对早期诊断治疗糖尿病肾病有重要临床意义。方法采用高效液相色谱法、免疫比浊法检测了117例糖尿痛患者与91例健康对照者体内的糖化血红蛋白与尿微量白蛋白的水平。按照HbAlc水平离低将这117例DM患者分为低值组(1丑)A1c〈7.O%),中值纽(7%≤HbAIC≤10%)和高值组(HbAlc〉10%),HbAlc水平与糖尿痛肾病发病率进行比较,探讨糖尿病肾痛发生与HbAlc高低之间的关系。结果糖化血红蛋白与尿微量白蛋白的水平在2型糖尿痛组高于健康对照组,差异有统计学意义,HbAlc水平与糖尿病肾病发病率成呈正相关(P〈0.01)。结论DM患者HbAlc水平和U—mAlb水平呈正相关关系,两者的水平与DM患者的早期肾脏损伤程度密切相关,二者联舍捡测对DN的早期诊断、早期治疗及监测有着重要意义。

Abstract:Objective To discuss the clinical significance of glycosylated hemoglobin Alc ( HbA1c ) and microalbumin - uria ( U - mAtb ) combined detection with diabetic nephropathy (DN). Methods HbAI c and U - mAlb in 117 patients with diabetes mellitus(DM) and 91 healthy individuals, as normal control were examined. The test of HbAlc was completed by High Performance Liquid Chromatography ( HPLC)and im-anme turbidity method was used to detect U - mAlb. According to HhAlc levels, the DM patients were divided into low (HbA1 c 〈 7.0% ), medium(7% ≤ HbAlc≤ 10%)and high value(HbAlc 〉 10% )groups. The differences indience of diabetic nephropathy among tile 3 groups were corn - pared. Results The levels of HbA1 c and U - mAlb were significantly higher in DM group than tht,se of normal control group( P 〈 0. 01 ). incidence of diabetic nephropathy presented a rising trend with the increase of HbAI c, which had a significant positive correlation( P 〈 0. 01 ). Conclusions In DM patients, HbA1 e levels and the levels of U - mAlb have a positive correlation, their levels are closely related to the early injure of kidney with DM. Combined detection of HbA1 c and U - mAlb has an important clinic significance to the early diagnosis, treat-ment and controlling of DN.
