摘 要: | 目的 观察系统性红斑狼疮(systematic lupus erythematosus,SLE)患者血清中25羟基维生素D3 [25(OH)D3]水平变化;25 (OH) D3与体内浆细胞样树突状细胞(plasmacytoid dendritic cell,pDC)数量、pDCs的表面分子及SEE疾病活动指数(SLE disease activity index,SLEDAI)相关性.方法 门诊收取符合条件的SLE患者46例和14例健康对照者的血清,SLE患者按治疗分为未治疗组、单用激素组、激素加羟基氯喹组,放射免疫法测定25(OH)D3水平,流式细胞术测外周血单个核细胞(peripheral blood mononuclear cell,PBMC)中pDCs水平和pDCs表面CD32、CD40、CD86、CD62L、CXCR4表达和改变,比较SEE患者与健康对照组、SEE不同治疗组间检测指标的差别.结果 SLE患者血清25(OH)D3水平较健康对照组低(t=3.06,P=0.003);SLE患者不同治疗组间25 (OH)13水平有统计学差异(F =4.62,P=0.015),未用药组低于激素组;25(OH)D3水平与SEE患者SLEDAI评分呈负相关(r=-0.38,P=0.050);在不同的25 (OH) D3浓度范围下,pDCs的水平、SLEDAI评分差异均有统计学意义(均有P <0.05);25 (OH) D3水平与3组SLE患者pDCs水平及表面分子均无明显相关性(均有P>0.05).结论 SLE患者血清25 (OH) D3水平低下,25(OH)D3水平对pDCs水平及疾病活动度可能有影响.

Abstract: | Objective To investigate the change of 25-hydroxy vitamin D3 [ 25 (OH) D3 ] and correlations of 25 (OH) D3 with plasmacytoid dendritic cells (pDCs), and expression of surface molecules and disease activity in patients with systemic lupus erythematosus. Methods Forty-six SLE patients who met eligibility of 1997 ACR classified criteria and fourteen healthy controls was enrolled in the study. SLE patients were divided into untreated, prednisone and hydroxychloroquine groups according to the therapy. 25 (OH) D3 in serum was measured using radioimmunoassay. The pDCs proportion and CD32, CIM0, CD86, CD62L, CXCR4 and expression on pDCs were detected by flow cytometry. The differences between SLE patients and healthy controls, different therapeutic groups were compared. Results The serum level of 25 (OH) D3 in SLE patients was significantly lower than that in healthy controls ( t = 3.06, P = 0. 003 ). The difference of 25 (OH) D3 level was statistically significant between three freatment groups( F = 4. 62, P = 0. 015). Untreated patients had significantly lower 25 (OH) D3 level than patients treated with prednisone alone. 25 (OH) D3 level correlated with SLE- DAI negatively (r = -0. 38, P =0. 050). The differences of pDCs proportion and SLEDAI were significant between three different 25 (OH) D3 concentrations ( all P 〈0.05). No significant correlation was found between 25 (OH) D3 level and pDCs proportion, and surface molecules expression in three therapeutic groups ( all P 〉 0.05 ). Conclusions SLE patients have a low level of 25 (OH) D3 ,which may influence pDCs proportion and disease activity. |
