老年人下呼吸道感染时实验室检查异常的临床评价

目的　探讨老年人下呼吸道感染时实验室检查异常与临床病情变化的关系。方法　总结 16 8例老年人下呼吸道感染时实验室生化检查结果 ,以是否有低磷血症把下呼吸道感染患者分为两组 (低磷组和非低磷组 ) ,比较两组的实验检查结果、治愈率及死亡率。结果　低磷血症组患者血清钠、钾、钙、总蛋白、白蛋白、胆固醇低于无低磷血症者 (P <0 0 5 ) ,尿素氮及碳酸氢根高于无低磷血症患者。而且 ,低磷血症组下呼吸道感染痊愈时间稍长于非低磷血症患者 ,死亡率也升高 (P <0 0 5 )。结论　下呼吸道感染时实验室检查异常与病情及预后不良有一定关系。

Study on the relationship between laboratory data and clinical presentation in elderly patients with lower respiratory tract infection

Objective To search for the factors closely associated with the progression and prognosis of lower respiratory tract infection in elderly patients.Methods The laboratory data obtained from 168 aged people with lower respiratory tract infection in ward were analyzed.The cases were divided into hypophosphatemia group(52 patients) and without hypophosphatemia group(116 patients) and the difference in laboratory data,the duration for infection resolution and the mortality rate were compared.Results The level of sodium,potassium,calcium,total protein,albumin and cholesterol in the hypophosphatemia group was lower than those in the without hypophosphatemia group(P<0.05).The resolution of lower respiratory tract infection in the hypophosphatemia group was slower than that in control group(without hypophosphatemia),and the mortality rate was higher in hypophosphatemia group compared with control group(P<0.05).Conclusion The hypophosphatemia,hypotassium,hyponatemia,hypocholestremia,and hypoproteinemia(abnormal laboratory data) can be used as predictors to indicate the severity and the prognosis of the aged patients with lower respiratory tract infection.
