指端容积脉搏曲线法与Swan - Ganz导管法心血管功能监测的临床对照研究

摘    要:目的:评价指端容积脉搏曲线法进行心血管功能监测的准确性和适用性。方法:对30例心脏外科术后血流动力学恢复稳定的患者,进行指端容积脉搏曲线法和Swan-Ganz导管法的心血管功能监测,观察心排出量、心脏指数、心搏指数、总外周阻力和外周血管阻力指标,并将两种方法所得结果进行比较。结果:两种方法测定的CO、CI及SI三项心血管功能数据结果无统计学差异(P>0.05),具有良好的直线相关性,相关系数r值分别为0.878、0.896和0.910(P<0.001);TPR与SVR也具有很好的直线相关性,相关系数r=0.823(P<0.001)。结论:指端容积脉搏法能够较为准确地监测外周循环接近正常的受试者的心血管功能状况,适用于此类人群的无创心血管功能监测和研究。

Abstract:Objective:To evaluate the veracity and applicability of the finger volume blood flow pulse method in cardiovascular function monitoring.Methods:Thirty post cardiac surgery patients had been studied oncardiovascular function changes with both the finger volume blood flow pusle method and the themodilution method.The CO,CI,SI and TVR from the two methods were compared by linear regression and t test.Results:There is no significant difference in CO,CI and SI between the two methods(P>0.05).There are high correlation in CO,CI,SI between the two methods,and their correlation coefficients are 0.878,0.896 and 0.910(P<0.001).The TPR was calculated by non-invasive blood pressure,while the SVR was calculated by invasive blood pressure.The TPR and SVR still showed the higher positive linear correlation with the correlative coefficient 0.823(P<0.001).Conclusion:Using the finger volume blood flow pulse method to determine the cardiovascular function changes in the people with peripheral circulation close to normal is practicable and reliable.
