掌侧钢板内固定治疗桡骨远端不稳定骨折22例

摘    要:目的探讨切开复位掌侧钢板内固定治疗桡骨远端不稳定骨折的手术方法及临床疗效。方法采用掌侧入路切开复位T形钢板内固定治疗桡骨远端不稳定骨折22例,术后石膏托制动1周开始腕关节功能训练。结果22例患者中,19例患者获得随访,随访4~18个月,平均10.4个月,骨折均在2个月内愈合。按照D inest腕关节功能评估标准,优13例,良4例,可1例,差1例,关节功能恢复优良率89.47%。结论掌侧钢板内固定治疗桡骨远端不稳定骨折,能较好恢复桡骨远端长度、掌倾角、尺偏角及关节面平整,手术操作方便,创伤小,腕关节功能恢复较快。

Abstract:Objective To explore the operative method and clinical effect of open reduction and volar plate internal fixation on the unstable distal radius fractures.Methods From March 2002 to October 2005,22 patients with unstable distal radius fractures were treated with open reduction and internal fixation using "T"titanium plate through volar approach.The functional exercise was carried out at 1 week after postopration with a cast.Results Nineteen cases were followed-up for 4~8 months,the average time was 10.4 months.All of the fractures were healed in 2 months.According to Dinest′s functional assessment system,eleven cases were exellent,four cases were good,one case was fair,one case was bad,the superordinary rate was 89.47%.Conclusion Open reduction and internal fixation using "T"titanium plate through volar approach is an effective method for the treatment of unstable distal radius fractures.It can ensure anatomical joint restoration by achieving most satisfied function of the joint,it is a minimal invasive surgery and easy to perform.
