外痔混切内痔高扎术治疗环状混合痔的临床研究

目的:研究探讨环状混合痔实施外痔混切内痔高扎术治疗的临床应用及效果。方法:回顾性研究我院2016年3月-2018年12月期间收治的环状混合痔患者78例的临床资料,根据患者手术适应症按照随机数字表法进行随机分组(研究组及对照组各39例),对照组给予吻合器痔上黏膜环切术(Procedure for Prolapse and Hemorrhoids,PPH)治疗,研究组则给予外痔混切内痔高扎术治疗,就两组患者治疗及预后效果进行观察对比。结果:两组患者在疗效及住院时间方面比较无显著差异(P>0.05),研究组手术时间显著短于对照组(P<0.05);研究组术后12 h的视觉模拟疼痛评分法(Visual Analogue Scale,VAS)评分显著高于对照组(P<0.05),但术后36 h及72 h两组VAS评分无显著差异(P>0.05);两组患者在术后并发症及随访不良情况方面比较无显著差异(P>0.05)。结论:对环状混合痔患者给予外痔混切内痔高扎术治疗的手术、预后及远期疗效均较高,且能够显著缩短手术时间,术后早期疼痛情况较显著,但疼痛改善也较为显著,是一类高效安全的治疗方式。

Treating annular mixed hemorrhoid by external hemorrhoids mixture operation plus internal hemorrhoids

Objective:To investigate clinical efficacy of external hemorrhoids mixture operation plus internal hemorrhoids on annular mixed hemorrhoid.Methods:78 cases were divided into 2 groups,39 cases in each.The control group was given PPH,and the study group was given external hemorrhoids mixture operation plus internal hemorrhoids.Results:There was no significant difference in efficacy and length of stay between groups(P﹥0.05).The operation time in the study group was significantly shorter than the control group(P﹤0.05).The 12-hour postoperative VAS score in the study group was significantly higher than the control group(P﹤0.05),but there was no significant difference in the VAS scores between groups at 36 and 72 hours post-operation(P﹥0.05).There were no significant differences in postoperative complications and adverse reactions between groups(P﹥0.05).Conclusion:External hemorrhoids mixture operation plus internal hemorrhoids on annular mixed hemorrhoid shows good effects,can significantly reduce operation time and improve postoperative pain.
