血液净化早期低血压发生率及其病位、病性证素与证候分布规律研究

目的研究血液净化早期低血压发生率及其病位、病性证素与证候分布规律。方法观察171例患者血液净化早期低血压的发生率,并分析早期低血压患者的病位、病性证素与证候分布规律。结果 171例患者发生血液净化早期低血压60例,发生率为35.10%,其中男性低血压发生率为36.10%,女性为33.30%,差别不大(P> 0.05)。60例早期低血压患者病位证素以脾为主,其次为肾。病性证素以阳虚、气虚为主,其次为血虚、血瘀、痰饮等。证候以虚证为主,其次为虚实夹杂证,最后为实证。虚证以脾肾阳虚为主,其次为脾肾气虚;虚实夹杂以气虚血瘀为主,其次为气虚水停;实证以痰瘀互结为主,其次为气滞血瘀。结论血液净化早期低血压发生率为35.10%;早期低血压病位证素分布呈脾>肾>心>肝>肺的趋势;病性证素分布呈阳虚>气虚>血虚>痰饮>血瘀>阴虚>气滞;证候分布呈虚证>虚实夹杂证>实证的趋势,其中虚证分布呈脾肾阳虚>脾肾气虚>心脾两虚的趋势,虚实夹杂证分布呈肾虚水泛>脾虚湿蕴>气虚血瘀的趋势,实证分布呈痰瘀互结>痰滞证>瘀滞证的趋势。

Study on the Incidence of Hypotension and Its Disease Location,Syndrome Elements and Syndrome Distribution in the Early Stage of Blood Purification

Objective:To study the incidence of hypotension in the early stage of blood purification and its disease location,disease syndrome elements and syndrome distribution. Methods:The incidence of hypotension in171 patients at the early stage of blood purification was observed,and analyzed for the disease location,disease syndrome elements and syndrome distribution. Results:Hypotension occurred in 60 of 171 patients at the early stage of blood purification. The incidence was 35.10%,36.10% in men and 33.30% in women,without significant difference(P > 0.05). In 60 cases of early hypotension,spleen was the main disease location,followed by kidney.The main disease syndrome element were Yang deficiency and Qi deficiency,followed by blood deficiency,blood stasis and phlegm. The syndrome was mainly deficiency syndrome,followed by deficiency and reality mixed syndrome,and finally empirical. Deficiency syndrome was mainly spleen kidney Yang deficiency,followed by spleen kidney Qi deficiency. Deficiency and reality mixed syndrome was mainly Qi deficiency and blood stasis,followed by Qi deficiency and water stagnation. The main syndrome was phlegm and blood stasis,followed by Qi stagnation and blood stasis. Conclusion:The incidence of hypotension in the early stage of blood purification is 35.10%.The distribution of each syndrome element shows the following trend. The syndrome elements of early hypotension:spleen>kidney>heart>liver>lung;disease syndrome elements:Yang deficiency>Qi deficiency>blood deficiency>phlegm>blood stasis>Yin deficiency>Qi stagnation;syndrome:deficiency syndrome>mixture of deficiency and reality syndrome>demonstration,among which,deficiency syndrome:deficiency of spleen and kidney Yang>deficiency of spleen and kidney Qi>deficiency of heart and spleen;deficiency and excess syndrome:kidney deficiency and water flooding>spleen deficiency and dampness accumulation>Qi deficiency and blood stasis;demonstration:phlegm and blood stasis syndrome>phlegm stagnation syndrome>blood stasis syndrome.
