真武汤合苓桂术甘汤应用于老年心功能衰竭患者临床治疗中的效果探讨

目的:探讨真武汤合苓桂术甘汤应用于老年心功能衰竭患者临床治疗中的效果。方法:观察2018年4月-2019年11月收治的78例老年心功能衰竭患者,随机分为西药组与中西结合组各39例,西药组运用常规西药治疗,中西结合组添加真武汤合苓桂术甘汤治疗,对比不同用药后患者治疗疗效、心功能有关指标情况。结果:在心功能评估方面治疗总有效率方面,中西结合组97.44%(38/39),西药组82.05%(32/39),差异有统计学意义(P <0.05);在患者中医证候治疗总有效率方面,中西结合组97.44%(38/39),西药组79.49%(31/39),差异有统计学意义(P <0.05);在患者治疗后1年内的再住院率方面,中西结合组12.82%(5/39),西药组28.21%(11/39),差异有统计学意义(P <0.05);在左心室射血分数(Left Ventricular Ejection Fraction,LVEF)、左心室舒张末期内径(Left Ventricular End Diastolic Diameter,LVEDD)、左心室收缩末期内径(Left Ventricular End Systolic Diameter,LVESD)、二尖瓣舒张早期最大血流速度/二尖瓣舒张晚期最大血流速度(E/A)、脑钠肽(Brain Natriuretic Peptide,BNP)等指标上,中西结合组各项指标治疗后改善程度明显多于西药组,差异有统计学意义(P <0.05)。结论:真武汤合苓桂术甘汤应用于老年心功能衰竭可以有效地改善疾病症状,治疗疗效更优,心功能改善更为明显,治疗综合情况更好。

Effect of the Zhenwu plus Linggui zhugan decoction in clinical treatment of the elderly patients with heart failure

Objective:To explore the effect of Zhenwu(真武汤)plus Linggui zhugan decoction(苓桂术甘汤)in clinical treatment of elderly patients with heart failure.Methods:A total of 78 elderly patients with heart failure who were treated from April 2018 to November 2019 were randomly divided into a western medicine group and a Chinese-western combination group,each group with 39 cases.The western medicine group was treated with conventional western medicine,while the Chinese-western combination group was additionally supplemented with Zhenwu plus Linggui zhugan decoction.Then the two groups were compared with regard to the therapeutic efficacy and cardiac function of patients following the two different medications.Results:In terms of evaluation of cardiac function,the total effective rate of Chinese-western combination and western medicine treatment and was 97.44%(38/39)VS 82.05%(32/39),(P<0.05);for the total effective rate of treatment of patients with TCM syndromes,Chinese combination treatment was 97.44%(38/39)and western medicine group 79.49%(31/39)(P<0.05).The rehospitalization rate within 1 year after treatment for the Chinese-Western combination group and the Western-medicine group was 12.82%(5/39)vs 28.21%(11/39)(P<0.05).Indicators such as LVEF,LVEDD,LVESD,E/A,and BNP in the Chinese-Western combination group after treatment improved much better than that in the western medicine group,(P<0.05).Conclusion:Zhenwu plus Linggui zhugan decoction can effectively improve the symptoms of the elderly patients with heart failure.Therefore,the better therapeutic effect and the more obvious improvement of Chinese medication for cardiac function made it better for the comprehensive treatment.
