中药离子导入与中药浴足治疗糖尿病周围神经病变效果比较

目的:比较中药离子导入与中药浴足治疗糖尿病慢性并发症周围神经病变(Diabetic Peripheral Neuropathy,DPN)的临床效果。方法:选择玉林市中医医院在2016年3月-2018年12月收住的130例多伦多临床神经病变评分(TCSS)均有8分以上的糖尿病伴有下肢周围神经病变的患者,随机分为三组,对照组38例,离子导入组50例,浴足组42例,离子导入组及浴足组都是在相同对照组的治疗基础上,同时在周围神经病变部位的皮肤给予中药离子导入或中药浴足治疗。结果:离子导入组与浴足组总有效率均优于对照组(P<0.05),而离子导入组与浴足组的总有效率差异无明显统计学意义(P>0.05);离子导入组治疗前后的各中医症状积分和总积分差值较对照组有非常显著下降(P<0.01),浴足组的差值较对照组有显著下降(P<0.05);离子导入组的腓总神经传导速度(MNCV)和腓肠神经传导速度(SNCV)提高幅度较对照组均有非常显著差异(P<0.01),浴足组的提高幅度较对照组有显著差异(P<0.05)。结论:中药离子导入和中药浴足治疗均能明显改善糖尿病周围神经病变临床症状,其中中药离子导入在改善中医症状及神经的运动、感觉传导速度较中药浴足更为显著,值得临床推广。

Efficacy comparison of TCM medicine iontophoresis and TCM medicine feet-bathing on diabetic peripheral neuropathy

Objective:To compare the clinical efficacy of TCM medicine iontophoresis and TCM medicine feet-bathing on diabetic chronic complications of peripheral neuropathy.Methods:130 patients were randomly divided into three groups,38 cases in the control group,50 cases in the iontophoresis group and 42 cases in the feet-bathing group.The iontophoresis group and the feet-bathing were treated by TCM medicine iontophoresis or a TCM medicine bath foot on the skin of the peripheral neuropathy based on the treatment of the control group.Results:The total efficiency in the iontophoresis group and the feet-bathing group were better than that in the control group(P<0.05),but the total efficiency in the iontophoresis group was not significantly different from the feet-bathing group(P>0.05).The difference of TCM symptom score and total score in the iontophoresis group was significant decrease than those in the control group(P<0.01),and the difference in the feet-bathing group was significantly lower than those in the control group(P<0.05),the increase of MCV and SCV in the iontophoresis group was significant difference from the control group(P<0.01),and the increase in the feet-bathing group was significantly different from the control group(P<0.05).Conclusion:TCM medicine iontophoresis and TCM medicine feet-bathing can significantly improve the clinical symptoms of diabetic peripheral neuropathy.Efficacy of TCM medicine iontophoresis is more effective in improvement of TCM symptom,nerve movement and sensory conduction velocity than TCM medicine feet-bathing,and the former is worthy of clinical promotion.
