添加中链甘油三酯和膳食纤维的肠内营养对腹部手术后患者营养及免疫的影响

目的研究添加中链甘油三酯(MCT)和膳食纤维的肠内营养对胃肠道术后患者营养及免疫功能的影响。方法选取40例消化道疾病行腹部手术的患者,随机分为两组,分别予添加中链甘油三酯(MCT)及膳食纤维的肠内营养和普通肠内营养,营养支持持续7 d,热卡为125.4 KJ/(Kg.d)。观察两组患者营养支持前后营养指标及免疫功能的变化。结果两组患者手术创伤后营养状况及免疫功能均有下降,营养支持后两组均有明显改善,与普通肠内营养相比,添加中链甘油三酯(MCT)和膳食纤维的肠内营养前白蛋白、T淋巴细胞计数及CD4/CD8水平均有明显增加。结论在消化道手术后患者,使用添加了中链甘油三酯(MCT)和膳食纤维的肠内营养能更好地改善机体的营养状况及免疫功能。

ENTERAL NUTRITION WITH MEDIAN CHAIN TRIGLYCERIDES AND DIETARY FIBER IN POSTOPERATIVE PATIENTS WITH DIGESTIVE TRACT DISEASE

Objectives To evaluate the effect of enteral nutrition with median chain triglycerides and dietary fiber in postoperative patients with digestive tract disease.Methods Forty postoperative patients with digestive tract disease were divided into two groups,who received either enteral diet with median chain triglycerides and dietary fiber or normal enteral diet for 7 days,nutritional and immunological parameters and were monitored.Results After surgical trauma,patients′ nutritional status and immunity were impaired.Compared with those who received normal enteral diet,patients who received enteral diet with median chain triglycerides and dietary fiber had higher prealbumine,T lymphocyte and CD_4/CD_8 level.Conclusions In postoperative patients with digestive tract disease,enteral nutrition with median chain triglycerides and dietary fiber can improve host nutritional status and immunity.
