食管癌、贲门癌术后复发再手术治疗

目的 总结食管癌和贲门癌术后吻合口复发癌再手术治疗的临床经验.方法 16例吻合口癌患者作再手术治疗,其中结肠食管颈部吻合8例、食管胃颈部吻合6例、食管胃胸顶吻合1例、开胸探查1例.结果 再手术死亡1例死亡率位6.3%,术后1、3、5年生存率分别为66.7%(10/15例)、26.7%(4/15例)、6.7%(1/15例).结论 对部分有适应证的患者,再手术是治疗食管癌、贲门癌术后吻合口癌行之有效的办法,早期发现、诊断、及时治疗能提高其疗效,延长生存期,改善饮食生活质量.

Reoperation for Recurrent Carcinoma of Esophagus and Cardia

Objective To evaluate the clinical experience of surgical treatment for anastomotic recurrence following resection of esophagea and cardia carcinoma.Methods Reoperation was perfomed in 16 patients with anastomotic recurrence.The surgical procedure performed included cervical esophagocolostomy in 8 cases,cervical esophagogastrostomy in 6 cases,esophagogastrostomy at the top of chest in 1,and explorative thoracotomy in 1.Results There was one reoperative death with mortality rate of 6.3%.The cause of death was respiratory failure,1-,3-and 5-year survival rates were 66.7%(10/15), 26.7%(4/15) and 6.7%(1/15).Conclusion Reoperation is still effective for some patients with anastomotic recurrence.Early diagnosis and prompt surgical treatment can improve the outcome and the quality of life.
