肾上腺素类血管活性药物在体外循环心脏术后的应用效果分析

目的分析肾上腺素类血管活性药物在体外循环心脏术后应用的效果.方法对2008年1月至2010年4月间的2482例体外循环心脏手术中,术后303例应用肾上腺素类血管活性药物患者资料进行回顾性分析.结果应用肾上腺素类血管活性药物的病例占体外循环心脏手术的12.2%.对不同种类心脏病手术,肾上腺素类药物应用的情况不同.绝大多数患者心功能改善;极少数因严重低心排出量综合征死亡.结论对体外循环心脏手术后心功能不全的患者,常规治疗循环维持不佳,应积极应用肾上腺素类药物;当心功能改善后,也需积极地、逐步地撤离肾上腺素类血管活性药物;肾上腺素类药物在心脏手术后应用的深层机制有待进一步研究。

Application of Adrenergic and Vasoactive Agents after Cardiac Operation with Extracorporeal Circulation

Objective To analyze the effect of application of adrenergic and vasoactive agents after cardiac operation with extracorporeal circulation.Methods We investigated retrospectively 303 cases of application of adrenergic and vasoactive agents in 2482 cases after cardiac operation with extracorporeal circulation from January 2008 to April 2010.Results The cases of application of adrenergic and vasoactive agents accounted for 12.2% of cases after cardiac operation with extracorporeal circulation.There was a difference in the usage rates of adrenergic and vasoactive agents in different heart operations.After application of adrenergic and vasoactive agents,heart functions of most patients were improved.Fewpatients died of serious lowcardiac output syndrome.Conclusions Application of adrenergic and vasoactive agents should be active to patients of cardiac insufficiency after cardiac operation with extracorporeal circulation thought regular treatments.When cardiac function is improved,adrenergic and vasoactive agents should be evacuated actively and gradually.The mechanism of application of adrenergic and vasoactive agents after cardiac operation with extracorporeal circulation should be further studied.
