适度扩肛预防直肠癌前切除术吻合口漏的发生

目的探讨适度扩肛对预防直肠癌前切除术后吻合口漏的作用。方法将375例直肠癌前切除术患者随机分为对照组（183例）和扩肛组（192例）,扩肛组术后48h开始扩肛,对照组术后未扩肛,比较两组术后吻合口漏发生情况。结果对照组186例发生吻合口漏11例,吻合口漏的发生率为6.01%;扩肛组189例发生吻合口漏3例,吻合口漏发生率为1.56%。两组相比,扩肛组吻合口漏的发生率较对照组低,其差异具有统计学意义,P=0.0456。结论适度扩肛对预防直肠癌前切除术后吻合口漏具有一定的作用。

Prevent anastomotic leakage by moderate anus dilatation for patients with rectal carcinoma afteranterior resection

Objective To investigate preventive effect of moderate anus dilatation to anastomotic leakage in patients with rectal carcinoma after anterior resection.Methods 375 cases of anterior resection of rectal cancer were randomly divided into control and postoperative anus dilatation group.Anus dilatation begin at 48 hours after the operation,to wear latex gloves and dip paraffin,lightly expansion match 3 of the anal canal distal to the mouth and continued to refer to about 2 minutes,1-2 times per day,7-9 days after operation.Results In control group of 186 cases of anastomotic leakage occurred in 11 cases,anastomotic leakage rate was 6.01%;anus dilatation group of 189 cases of anastomotic leakage occurred in 3 cases,anastomotic leakage rate of 1.56%.The incidence of anastomotic leakage lower in anus dilatation group than in the control group,the difference was statistically significant（P=0.0456）.Conclusion Moderate anus dilatation can prevent anastomotic leakage after anterior resection.
