中西医结合治疗糖尿病足45例疗效观察

目的观察在常规西医治疗基础上应用自拟复元汤治疗糖尿病足的临床疗效。方法将86例糖尿病足患者随机分为2组,治疗组45例在常规西医治疗基础上应用自拟复元汤,对照组41例予常规西医治疗。2组均4周为1个疗程,2个疗程后观察2组临床疗效,治疗前后血糖、尿糖、血脂、血液流变学和患肢动脉彩色多普勒超声血流峰值流速变化。结果治疗组总有效率95.56%,对照组总有效率82.93%,2组总有效率比较差异有统计学意义（P〈0.05）,治疗组疗效优于对照组。治疗组治疗后空腹血糖、早餐后2 h血糖、24 h尿糖定量、纤维蛋白原、血浆黏度、全血黏度（高切）、血清总胆固醇及甘油三酯均低于对照组（P〈0.05,P〈0.01）。治疗组治疗后腘动脉、胫后动脉彩色多普勒超声血流峰值流速均高于对照组（P〈0.05）。结论在常规西医治疗基础上应用自拟复元汤治疗糖尿病足,疗效优于单纯西医治疗。

Clinical observation on 45 cases of diabetic gangrene treated by the combination of traditional Chinese medicine and western medicine

Objective To study clinical effect on diabet gangrene with the therapy of combination of traditional Chinese medicine and western medicine.Methods 86 patients with diabetic gangrene were randomly divided into two groups.Treatment group were treated with the combination of Fuyuan decoction and routine western Medicine,and control group were treated by routine western medicine.The course was four weeks in two groups.The therapeutic effect was compared after two courses.Changes of blood glucose,urine glucose,hemorheology,ultrasound blood flow peak velocity of lower limbs artery were recorded.Results The total effective rate of treatment group was 95.56% and control group was 82.93% after treatment.FPG,2h plasma glucose after breakfast,24 h urine glucose quantitative,fibrinogen,plasma viscocity,TG in treatment group was lower than those in control group after treatment（P0.01,P0.05）.The ultrasound blood flow peak velocity of lower limbs artery in treatment group was higher than that in control group after treatment（P0.05）.Conclusion The effect of Fuyuan decoction combined routine western medicine was superior to that routine western medicine on the treatment of diabetic gangrene.
