诺舒阻抗控制子宫内膜切除系统与左炔诺孕酮宫内缓释系统治疗功能失调性子宫出血的疗效对比

目的：比较诺舒阻抗控制子宫内膜切除系统（诺舒）及左炔诺孕酮宫内缓释系统（曼月乐环）治疗功能失调性子宫出血（功血）的疗效及安全性。方法各56例功血患者分别采用诺舒（诺舒组）或曼月乐环（曼月乐组）治疗。术后1、3、6、12个月观察两组患者疗效、血红蛋白水平、月经量评分（ PBAC）、子宫内膜厚度、满意度及并发症发生率。结果诺舒组术后1、3、6、12个月有效率、闭经率均明显高于曼月乐组（P＜0．05）；诺舒组满意度明显高于曼月乐组（P＜0．05），并发症发生率明显低于曼月乐组（P＜0．05）；两组患者血红蛋白水平均随着时间的增加呈升高趋势（P＜0．05），术后诺舒组血红蛋白水平高于曼月乐组（P＜0．05）；术后诺舒组PBAC评分及子宫内膜厚度均低于曼月乐组（P＜0．05）。结论采用诺舒行子宫内膜切除术治疗功血具有创伤小、并发症少、近期疗效好等优点。

Comparison of efficacy between NovaSure endometrial ablation system and levonorgestrel-releasing intrauterine system for treatment of dysfunctional uterine bleeding

Objective To compare the efficacy and safety between NovaSure endometrial ablation system ( abbreviated to NovaSure ) and levonorgestrel-releasing intrauterine system ( Mirena ) for the treatment of dysfunctional uterine bleeding .Methods Treatment with NovaSure(NovaSure group) and Mirena(Mirena group) were performed in each 56 patients with dysfunctional uterine bleeding.The efficacy, hemoglobin level,score of pictorial blood loss assessment chart(PBAC),endometrial thickness,satisfaction and incidence of complications were observed at 1,3,6,12 months after operation.Results At 1,3,6,12 months after operation,the effective rate and the proportion of patients with amenorrhea in NovaSure group were significantly higher than those in Mirena group(P<0.05).The satisfaction was significantly higher and the incidence of complications was significantly lower in NovaSure group compared to Mirena group (P<0.05).The hemoglobin levels of both groups obtained an increase trend over time(P<0.05),and the hemoglobin level after operation in NovaSure group was higher than that in Mirena group (P<0.05).The score of PBAC and the endometrial thickness after operation in NovaSure group were less than those in Mirena group (P<0.05).Conclusion For dysfunctional uterine bleeding ,endometrial ablation with NovaSure has the advantages of less trauma , fewer complications and better short-term efficacy.
