肥胖2型糖尿病证候分布特点及其与糖脂代谢的相关性

目的：探讨肥胖2型糖尿病的中医证候分布规律及其与血糖、血脂的相关性。方法根据是否患2型糖尿病，将179例肥胖患者分为肥胖非糖尿病组和肥胖糖尿病组，采集四诊信息进行中医辨证分型，同时测定血糖、血脂，分析中医证候与糖脂代谢相关性。结果肥胖非糖尿病组和肥胖糖尿病组证候分布有统计学意义（P ＜0.05）。肥胖非糖尿病组以痰湿证为主，肥胖糖尿病组以郁热证为主。阴虚证与 FPG 呈正相关（r =0.232，P =0.030），与 HDL-C 呈负相关（r =-0.211，P =0.048）；结热证与 TG 呈正相关（r =0.215，P =0.044），与 FPG 呈负相关（r =-0.236，P =0.027）；TG 与气郁证呈负相关（r =-0.233，P =0.029）。结论肥胖2型糖尿病患者以郁热证为主，（肝胃）郁热证是其主要证型。阴虚证、结热证、气郁证与血糖、血脂水平存在相关性。

Correlation analysis of TCM syndrome distribution and glucose and lipid metabolism in patients with obese type 2 diabetes mellitus

Objective To investigate the TCM symptomatology characters of obese type 2 diabetes mellitus and correlations between TCM syndrome types and blood glucose and lipid. Methods The 179 cases of obese were divid-ed into two groups according whether they were diabetes mellitus. Four diagnostic informations were collected and TCM syndromes were determined. Meanwhile blood glucose and lipid were measured,and correlations between TCM syn-drome types and blood glucose and lipid were analyzed. Results There was significant difference on syndromes be-tween the two groups(P < 0. 05),and the obese type 2 diabetes mellitus group was dominated by phlegm-dampness syndrome and the other group by heat twisting syndrome. Yin deficiency syndrome correlated positively with FPG (r = 0. 232,P = 0. 030),and correlated negatively with HDL-C(r = - 0. 211,P = 0. 048). Heat twisting syndrome correlated positively with TG(r = 0. 215,P = 0. 044),and correlated negatively with FPG(r = - 0. 236,P = 0. 027). Qi stagnation syndrome correlated negatively with TG(r = - 0. 233,P = 0. 029). Conclusion The main syndrome of obese type 2 diabetes mellitus was heat twisting syndrome,and the heat twisting syndrome in liver and stomach was the main syndrome. There was a correlation between Yin deficiency syndrome,heat twisting syndrome and Qi stagnation syndrome and blood glucose and lipid.
