小血管张力测量与小血管减张缝合技术研究

摘    要:
目的 制备小因管张力计测量小血管断端张力,并探讨减张缝合法在小血管损伤中的作用。方法 采用手术于套橡胶条小血管张力计并应用于观察离体猪桡动脉吻合口张力大小与通畅情况的关系。测量小血管断是张力大小,并根据情况应用两断端间血管壁上缝减张线的减张缝合法治疗106例小血管损伤。结果 小血管吻合口张力大小与吻合成功明显有关,张力小于10^6时直接套入可获成功,张力大于150时即使减张缝合也很难保证吻合口通畅

Abstract:
Objective To measure the tensile strength in the broken ends of small blood vessels using mini vascular tensometer,and to evaluate relaxation suture for the treatment of small vessel damage.Methods Mini vascular tensometer was made with rubber strips of surgical gloves.The tensometer was at first used to measure the in vitro tension in anastomotic stoma of porcine radial artery to determine its relationship with free blood circulation.106 patients with small vascular damage were treated with relaxation suture.Results The tension in anastomotic stoma of small vessel was notably related with the success of anastomosis.When the tension was less than 10 degree,the broken vessel can be cuffed directly.When the tension was more than 150 degree,it is difficult to ensure the unobstructed anastomotic stoma by using even the relaxation technique;other methods should be resorted to.Excellent and good results were obtained in this series of 106 cases of small vessels damage with tension between 10 and 150 degrees following treatment with relaxation suture.Conclusion This mini vascular tensometer is simple in preparation and handy.Relaxation suture relieved the tension of the suture site and thus improved the quality of anastomosis.
