枢丹改进方案预防顺铂所致恶心呕吐的临床观察

目的：观察比较枢丹改进方案（单剂量枢丹联合地塞米松及胃复安）和单剂量枢丹预防顺铂方案联合化疗引起恶心呕吐的临床疗效。方法：将８９例接受以顺铂为主联合化疗患者随机分成２组，枢联组５０例接受６７周期化疗，止吐方案为化疗前２０ｍｉｎ静脉注射地塞米松１０ｍｇ及胃复安２０ｍｇ，化疗前１０ｍｉｎ再静脉注射枢丹８ｍｇ。枢丹组３９例接受５２周期化疗，化疗前１０ｍｉｎ静脉注射枢丹８ｍｇ，观察第１～５天恶心、呕吐控制率并进行比较。结果：枢联组对急性恶心呕吐控制率分别为８３．６％及９４．１％均较枢丹组（７５％及８１．２％）明显提高（Ｐ＜０．０１及Ｐ＜０．０５），对延缓性呕吐控制率亦优于枢单组（Ｐ＜０．０５）。结论：枢丹改进方案可有效地预防顺铂所致急性恶心呕吐，对延缓性呕吐也有一定疗效。用法简便，不良反应轻微，经济实用，值得推广使用

Modified Zudan Regimen in Prophylaxis of Nausea and Vomiting by Cisplatin

Objective:The effect of Zudan with modified regimen (combined use of one dose of each of Zudan, dexamethasone and metoclopromide) and sigle dose of Zudan was compared in the prevention of DDP induced emesis.Methods:89 patients were randomized into two groups: group 1,50 patients received 67 cycles of chemotherapy. They were given dexamethasone 10mg and metoclopramide 20mg iv, 20 minutes before DDP and Zudan 8mg iv, 10 minutes before DDP. Group 2, 39 patients received 52 cycles of chemotherapy. They were given Zudan 8mg iv,10 minutes before DDP. Results: The control rate of acute nausea and vomiting in group 1 were 83.6% and 94.1% compared to 75.0% and 82.1% in group 2 ( P <0.01 and P <0.05). Conclusion: Zudan with modified regimen is more effective in the control of DDP induced emesis than Zudan given alone.
