手术室医护一体化管理在降低骨科手术感染风险中的应用

目的观察手术室实施医护一体化管理模式在降低骨科手术感染风险中的应用效果。方法将我院480例骨科手术患者采用随机数字表法分为观察组与对照组,各240例。对照组采用常规院内手术室护理管理模式,观察组采用手术室医护一体化管理模式。通过比较2组护理手术室不良事件发生和术后切口感染情况,评价医护一体化管理模式对降低骨科手术感染风险的效果。结果观察组在物品准备缺陷率、抗生素应用不合格率、外科手消毒监测不合格率、手术部位皮肤消毒监测不合格率及总不良事件发生率5个方面均明显低于对照组,2组比较差异有统计学意义(P<0.05或P<0.01);对照组手术后感染10例,感染发生率为4.2%(10/240),观察组手术后感染2例,感染发生率为0.8%(2/240),2组比较差异有统计学意义(P<0.05)。结论手术室医护一体化管理模式可有效降低骨科手术感染风险及手术室不良事件发生率,具有广泛的临床推广价值。

Application of doctor-nurse integration management in operating room in reducing the risk of infection of orthopedic surgery

Objective To observe the application effect of doctor-nurse integration management model in reducing the risk of infection in orthopedic surgery. Methods Totally 480 cases of orthopedic surgery patients were divided into observation group and control group,with 240 cases in each. The control group adopted the routine hospital operating room nursing management model,and the observation group adopted doctor-nurse integration management model. By comparing the incidence of nursing adverse events and the postoperative incision infection rate between the two groups,this paper evaluates the effect of doctor-nurse integration management model in reducing the risk of infection in orthopedic surgery. Results The articles preparation defect rate,unqualified rate of antibiotic application,unqualified rate of surgical hand disinfection monitoring,unqualified rate of skin disinfection monitoring of operation site and incidence of total adverse events of the observation group were lower than those of the control group,and the difference was statistically significant between the 2 groups( P<0.05 or P<0.01). Ten cases caught postoperative infection in the control group with the incidence of postoperative infection 4. 2%( 10/240),2 cases caught postoperative infection in the observation group with the incidence of postoperative infection 0.8%( 2/240),and the difference was statistically significant between the 2 groups( P < 0. 05). Conclusion It can effectively reduce the postoperative infection rate of orthopedic surgery and the incidence of adverse events in the operating room to use the management model of doctor-nurse integration,which has wide clinical promotion value.
