不同中医证候心力衰竭患者的心理状况分析

目的探讨不同中医证候心力衰竭患者心理问题的异同,以指导临床护理人员进行相应的心理干预。方法应用症状自评量表(SCL?90)对218例心力衰竭住院患者以问卷方式进行心理调查,同时在中医心血管病专家的指导下对心衰患者进行中医证候诊断,并进行相关性分析。结果①心力衰竭患者存在明显的心理问题;②不同中医证候心力衰竭患者心理问题表现不同;③以“实证”为主要证候与以“虚证”为主要证候的心力衰竭患者在强迫症状、焦虑、敌对、偏执方面的表现有差异。结论临床护理人员应重视心力衰竭患者的心理护理,并根据不同中医证候诊断,应用中西医心理护理知识给予相应干预,以提高心力衰竭患者的心理健康状况。

Psychological analysis of heart failure patients with different Chinese medicine syndromes

Objective To explore the relativity between psychological symptoms based the Symptom Checklist 90 (SCL-90) investigation and traditional Chinese medicine(TCM) syndrome of the patients with heart failure.Methods In this study, 218 inpatients with heart failure were investigated with Symptom Checklist 90 (SCL-90) and were divided into two groups according to their TCM syndrome diagnosed by TCM experts, i.e. excess syndrome or deficiency syndrome. And the relativity between psychological symptoms and TCM syndrome of the patients was analyzed. Results There were obvious psychological problems among cardiac failure patients. Patients with different TCM syndromes showed different psychological symptoms such as forced symptoms, anxiety, hostility and paranoia with significant differences.Conclusion Clinical nurses should attach importance to psychological nursing of patients with heart failure, and take corresponding measures according to their TCM syndrome.
