小切口白内障囊外摘除联合人工晶状体植入术治疗硬核白内障67例临床分析

【目的】分析小切1：2白内障囊外摘除术（ECCE）和超声乳化吸出术（Phaco）治疗硬核白内障的疗效和并发症。【方法】选择硬核白内障患者123例（162眼），分为ECCE组（67例89眼）和Phaco组（56例73眼），分别行小切口白内障囊外摘除联合人工晶体植入术、白内障超声乳化吸出联合人．7--晶体植入术，观察3个月，比较两组患者术后视力改善情况及术后并发症。【结果】全部病例术后视力均较术前提高，术后1d、3d时两组视力比较，ECCE组好于phaco组，差异有统计学意义（P〈O．05）。术后1周，1个月、3个月时两组视力比较，差异无统计学意义（P〉0．05）。ECCE组患者术后角膜水肿发生率低于Phaco组，差异有统计学意义（P〈0．05）。而两组其他并发症比较，差异无统计学意义（P〉0．05）。【结论】小切口白内障囊外摘除联合人工晶体植入术早期视力恢复好，术后角膜水肿并发症少，是治疗硬核白内障的有效方法。

Clinical Analysis of the Effect of Extracapsular Cataract Extraction Combined with Implantation of Artificial Lens in the Treatment of Hard Nucleus Cataract

[Objective] To analyze the efficacy of extracapsular cataract extraction（ECCE） and phacoemulsification （Phaco） in the treatment of hard nucleus cataract and its complications. [Methods] Totally 162 eyes of 123 patients with hard nucleus cataract were selected and divided into ECCE group（n= 67） and Phaco group（n= 73）. The patients in two group underwent ECCG combined with implantation of artificial lens and Phaco combined with implantation of artificial lens, respectively. All patients were observed for 3 months. The visual acuity and the main complications were compared between two groups. [Results] Visual acuity of all eyes were improved after operation. At 1 and 3 days after operation, the visual acuity of ECA]E group was better than that of phaeo group, and there was significant difference（ P 〈0.05）. At 1 week, 1 month and 3 months after operation, there was no significant difference of the visual acuity between the two groups（ P 〉0.05）. The incidence of corneal edema in operation of the ECCE group was lower than that of the phaco group, and there was significant difference（ P 〈0.05）. There was no significant difference of other complications after operation between the two groups（ P 〉0. 05）. [Conclusion] ECCE combined with implantation of artificial lens has quic- ker recovery of visual acuity and less corneal edema complication, so it is effective for the treatment for hard nucleus cata- ract.
