后腹腔镜肾蒂淋巴管结扎术治疗乳糜尿疗效观察

目的:探讨后腹腔镜肾蒂淋巴管结扎术治疗乳糜尿的手术方法和临床效果。方法:12例患者,男10例,女2例;年龄48~66岁。行后腹腔镜肾蒂淋巴管结扎术,观察手术时间、术中出血量、术后肠道功能恢复和术中术后并发症及手术效果。结果:手术平均时间107 min,术中平均出血量48 mL,术后肠道功能恢复时间24~48 h,术后当日乳糜尿消失,术后平均住院6.2 d,术中术后无明显并发症。随访2~24个月,无复发。结论:后腹腔镜肾蒂淋巴管结扎术治疗乳糜尿近期疗效较好,具有术中出血量少、微创、淋巴管结扎彻底、术后住院时间短、恢复快等优点,是目前治疗乳糜尿较理想的手术方式。

Retroperitoneoscopic Ligation of Renal Lymphatic Vessels for the Treatment of Chyluria

Objective:To evaluate the operation technique and the clinical efficacy of retroperitoneoscopic ligation of renal lymphatic vessels for the treatment of chyluria.Methods:Twelve patients with chyluri,including male 10 and female 2 in 46 to 66 years old,underwent retroperitoneoscopic ligation of renal lymphatic vessels.The operation time,blood loss,postoperative intestinal function recovery,complications during operation and the operative efficacy were observed.Results:The average operation time was 107 min,the average blood loss 48 ml and the postoperative intestinal function recovery time 24 to 48 h.Chyluria disappeared on the day of operation.The average hospital stay after operation was 6.2 days.No complications occurred during operation.A follow-up for 2 to 4 months showed no recurrence.Conclusion:Retroperitoneoscopic ligation of renal lymphatic vessels for the treatment of chyluria is an effective and efficient surgical procedure and has advantages of less blood loss,minimal invasion,complete lymphatic vessels ligation,short hospital stay and rapid recovery.So far,it is an ideal surgical procedure for treatment of chyluria.
