MCV、MCH在产前筛查地中海贫血中的应用价值

目的 探讨红细胞平均体积(mean corpuscular volume,MCV)、平均红细胞血红蛋白量(mean corpuscular hemoglobin,MCH)、并结合血红蛋白电泳HbA2检测结果在筛查地中海贫血中的应用价值.方法 采用全自动血细胞分析仪分别对200例MCV＜80 fl、MCH＜26 pg实验组和200例MCV＞80 fl、MCH＞26 pg的正常对照者进行血红蛋白电泳测定HbA2,对结果作相关统计学分析,分析MCV、MCH作为地中海贫血筛查方法的敏感度和特异度.结果 实验组筛查出地中海贫血的占58.5%,对照者筛查出地中海贫血占2%,实验组MCV、MCH与对照组比较有统计学意义(t=18.214,P＜0.01).结论 MCV、MCH可作为筛查地中海贫血的有效指标,可疑者建议做进一步的检查,以明确诊断.

The value of MCV and MCH on screening thalassanemia in prenatal diagnosis

Objective To explore the clinical value of mean corpuscular volume(MCV),mean corpuscular hemoglobin(MCH),hemoglobinA2(HbA2)'s result in hemoglobin electrophoresis joint detection in the prenatal diagnosis of Thalassemia. Methods MCV and MCH were detected by automatic blood cell analyzer,two hundred samples with MCV ＜ 80fl, MCH ＜ 26pg as experimental group and two hundred samples with MCV ＞ 80fl,MCH ＞ 26pg as control group were tested the HbA2 by hemoglobin electrophoresis. The correlation analysis was performed. To analysis the sensitivity and Specificity of the MCV and MCH as the screening method of the Thalassanemia. Results Of the 200 cases, 58.2% were detected for the Thalassanemia in the experimental group,2% were detected for the Thalassanemia in the control group. There were significant differences between the experimental group and the control group(t = 18. 214, P ＜ 0.01). Conclusion MCV and MCH could screen the Thalassanemia as effective index. Suggestted the suspects should do further examination for explicit diagnosis.
