儿童AD的中医证候特征及中药临床疗效

目的分析儿童特应性皮炎(AD)中医证候特征及中药临床疗效。方法选取我院2014年6月—2016年3月收治儿童AD患者180例为研究对象,依照数字随机法分为对照组和研究组各90例,对照组患者采用地氯雷他定干混悬剂治疗,研究组患者在西药治疗基础上采用中医辨证治疗,分析AD患儿中医证候特征和治疗效果。结果患儿多表现为脾虚湿蕴型、湿热蕴结型,其次为血虚风燥型。研究组患者治疗总有效率(84.4%)显著高于对照组(55.6%)(P0.05)。经过治疗,两组患者SCORAD评分均显著降低(P0.05),研究组患者SCORAD评分(29.61±12.37)分显著低于对照组(38.52±10.29)分(P0.05)。两组患者EOS计数均显著改善,研究组EOS水平显著低于对照组(P0.05),研究组Ig E(115.32±63.27)Iu/ml显著低于对照组(144.25±102.34)Iu/ml(P0.05)。研究组4例患儿出现腹泻,随症中药加减后症状消失。对照组5例出现嗜睡,不影响治疗。结论儿童AD中医证型复杂,多表现为脾虚湿蕴型、湿热蕴结型,中药治疗血虚风燥型儿童AD能够改善患者各项症状及生活质量,安全性高。

TCM Syndrome Characteristics of Children with Atopic Dermatitis and Clinical Effect of Chinese Herbal Medicine

Objective To analyze TCM syndrome characteristics of children atopic dermatitis ( AD) and clinical effect of Chinese herbal medicine, and to provide reference for clinical treatment .Methods In our hospital from June 2014 to March 2016, 180 cases of children with AD were selected , and they were randomly divided into study group and control group in accordance with the numbers , with 90 cases in each group.The control group took desloratadine dry suspension therapy .The study group was treated with TCM syndrome differentiation therapy and Western medicine .TCM syndrome characteristics of children with AD and clinical effect were analyzed .Result The results showed syndrome characteristics of children with AD were dampness stagnancy due to spleen deficiency type , accumulated dampness-heat type , and followed by blood deficiency and wind-dryness .The total effective rate of the study group ( 84.4%) was significantly higher that of the control group (55.6%) (P<0.05).After treatment, SCORAD scores of the two groups were significantly decreased (P<0.05).The SCORAD score of the study group patients was (29.61 ±12.37) which was significantly lower than (38.52 ±10.29) of the control group (P<0.05).The EOS counts of the two groups were significantly improved , the EOS of the study group was significantly lower than that of the control group ( P<0.05).The IgE of the study group was (115.32 ±63.27) Iu/ml which was significantly lower than (144.25 ±102.34) Iu/ml of the control group (P<0.05).There were 4 cases of children with diarrhea in the study group , and symptoms disappeared with drug withdrawal .There were five cases of drowsiness in the control group , and did not affect the treatment .Conclusions TCM syndrome of children with AD is complicated , and the performances are dampness stagnancy due to spleen deficiency type and accumulated dampness -heat type .Chinese herbal medicine in the treatment of children with AD can improve the symptoms and quality of life , and has high safety .
